🩺 Your Role as a CNA
As a CNA at Heart To Serve Home Care LLC, you are the heart and hands of our mission. Your care helps clients live safely, comfortably, and with dignity in their own homes.
Duties include:
Duties include:
- Personal Care Assistance: Bathing, dressing, grooming, toileting, and hygiene.
- Safe Transfers & Mobility: Support ambulation and prevent falls.
- Emotional & Social Support: Be a companion, a listener, a comforting presence.
- Monitoring & Reporting: Stay observant—report changes to the RN.
- Meal Preparation & Nutrition: Plan and cook meals to meet dietary needs.
- Medication Reminders: Help clients stay on schedule with their meds.
- Light Housekeeping & Home Safety: Keep the home clean and hazard-free.
- Dementia & Alzheimer’s Care: Use gentle redirection and structure for memory care clients.
- Family Communication: Provide updates and reassurance when needed.
- Documentation & Compliance: Maintain care logs and follow HIPAA guidelines.

- Social Security Card or Birth Certificate
- Current CPR Certification
- Negative TB Test or Chest X-ray
- Clean Criminal Background Check
- Reliable Transportation to client homes
