Home Health Admission Registered Nurse - 1.0 FTE

JOB SUMMARY: The Admissions Nurse is responsible for conducting the initial home health admission in a patient’s home or in a health care facility. This position works with the intake team, referring physician and other referral sources.
ESSENTIAL DUTIES AND RESPONSIBILITIES
· Conducts initial home health admission for all new home health patient referrals.
· Communicates with the Intake Department and the referring physician to assure appropriateness of each referral and those necessary certifications are obtained.
· Completes comprehensive oasis assessment at time of admission.
· Develops accurate and individualized POC at the time of admission and ongoing.
· Ensures all home health admission documentation is completed at time of admission.
· Develops appropriate care plans for the patient’s overall condition at the time of admission.
· Keeps the Intake Department informed about progress of referral/admission.
· Makes appropriate referrals for other discipline services and medical equipment/supply companies to meet patient care needs in a timely manner.
· Coordinates care of patients with a variety of disciplines and community resources, both internal and external in a fiscally responsible manner.
· Completes and submits documentation within agency timeliness policies that is complete, legible, and clearly reflects care given.
· Provides hands on nursing care for a diverse group of patients as assigned.
· Utilizes appropriate resources to maintain knowledge and skills for the home health care in all settings including regulatory/reimbursement guidelines and integrates into clinical practice.
· Uses critical thinking skills which positively affect individual patient outcomes.
· Assesses for, provides, and documents patient and family teaching related to patient’s
health situation on an ongoing basis (i.e., medications, disease processes, treatments, etc.) and provides written teaching information as appropriate.
· Updates Home Health Clinical Manager regarding status of patients on regular basis, including a consultation for any unusual circumstances. Assesses for, provides, and documents staff teaching related to patient care on an ongoing basis when patient resides in a facility.
· Ensures all documentation included in the medical record is thorough, complete and accurate, requiring minimal corrections from supervisor when reviewed.
· Documents evidence of coordination and collaboration when patient resides in a facility.
· In collaboration with patient care conferences, determines need for other disciplines (aide, PT, OT, ST, SW) or service and notifies staff.
· Works as Home Health visit nurse when no admissions are available.
· Meets productivity standards as annually established.
· Other duties as assigned

Minimum Qualifications
· BSN or minimum of two years of home health care experienced preferred.
· Minimum of two years of clinical experience.
· Current Wisconsin RN license required.
· Current driver's license.
· Able to take on call responsibilities on rotating basis.
· Excellent communication skills.

Physical Requirements
Medium work: Job involves standing more than four hours per day; stooping, bending, lifting/pushing up to 25 lbs. etc. are regularly required.
