Description

Nurse Supervisor


The Nurse Supervisor provides a balanced combination of direct patient care, staff supervision, and team-level operational leadership to ensure safe, high-quality clinical services and effective team performance. This role maintains a strong clinical presence while serving as a visible supervisor and operational leader who supports staff development, workflow effectiveness, and alignment with organizational standards.


Region/Location:  Green Bay East & St Bay 


Schedule:  5 days per week (variable),  every 3rd weekend max (currently at every 8th weekend), on call rotation - 10-15 times per Year, 1-2 holidays per year.


Job Duties:


Direct Patient Care

  • Deliver direct patient care in accordance with RN job description (refer to separate RN job description), licensure requirements, professional standards, and organizational policies.
  • Maintain current clinical skills and competencies relevant to assigned patients, programs, and populations served.
  • Collaborate with interdisciplinary team members to ensure coordinated, patient-centered care.
  • Complete accurate, timely, and compliant clinical documentation.
  • Identify clinical trends, risks, or resource needs and communicate findings to the Operations Manager or Educator.
  • Participate in case conferences, interdisciplinary meetings, and care planning discussions.
  • Support patient coverage during periods of high census, staffing shortages, or unplanned absences.
  • Participate in required weekend, holiday, and on-call rotations in accordance with organizational policy.

Staff Supervision

  • Provide real-time coaching, mentorship, and clinical or behavioral guidance to assigned staff.
  • Engage in regular leader rounding and check-ins with all team members. 
  • Coach, mentor, and guide performance or behavioral concerns. Escalate significant concerns to the appropriate leader. 
  • Intervene promptly in staff conflicts, facilitate respectful dialogue, and reinforce behavioral expectations.
  • Support onboarding and acclimation of new staff, ensuring role clarity and smooth integration.
  • Partner with the Educational Services leaders to identify learning needs and support targeted training or follow-up education.
  • Prepare for and conduct annual EMPOWER meetings in accordance with organizational guidelines.

Team Operations & Development Leadership

  • Lead or support daily huddles and weekly POD meetings to promote operational alignment and readiness.
  • Serve as a primary clinical escalation resource for frontline staff, providing real-time guidance and coordinating with education leadership as needed.
  • Foster a positive team culture focused on collaboration, accountability, engagement, and recognition.
  • Share productivity, performance, and workflow trends with staff to promote awareness and efficiency.
  • Implement approved workflow or process changes, providing clarification and reinforcement to ensure consistent practices.
  • Identify and communicate workflow barriers, inefficiencies, or safety concerns to leadership.
  • Gather staff input and communicate feedback to appropriate leaders to support continuous improvement initiatives.
  • Partner with Education and Quality teams on process rollouts or training updates.

 Knowledge, Skills, and Abilities:

  • Computer proficiency (MS Office – Word, Excel, Electronic Medical Record, Scheduling System, and Outlook).
  • Ability to respond to dynamic, high-acuity situations.
  • Highly motivated and organized with a strong attention to detail and analytical skills to help solve problems.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to maintain confidentiality and discretion in business relationships and exercise sound business judgment. 
  • Ability to keep up to date with policies, evidence-based best practices, and regulations.
  • Ability to prioritize and handle multiple tasks and capable of dealing with deadline pressures.
  • Comply with all applicable local, state, and federal laws and regulations as well as policies and procedures of Unity.
  • Satisfy mandatory training requirements regarding local, state, and federal regulations and/or Unity’s policies and procedures.
  • Perform other job-related duties as assigned. 
  • Ability to adjust to frequently changing demands.

Requirements

Minimum Qualifications & Certification/Licensure:

  1. Associate degree in Nursing required, bachelor’s or master’s degree preferred.
  2. RN license in the state of Wisconsin required (RN must practice under the law and regulations governing nursing in the state of Wisconsin). 
  3. Minimum 3 years of clinical nursing experience.
  4. Prior experience in home health, hospice, or facility/community-based care.
  5. Prior experience leading others, fostering a positive team environment, problem-solving and decision- making skills, and an effective communicator. 
  6. Valid Wisconsin Driver’s License, reliable transportation, and proof of current automobile insurance coverage.
