Care Coordinator
Healthy At Home Caregivers | Full-Time
Dana Point, CA | In-Office + Limited Field Work
Compensation: $46,000 – $54,000 annually (DOE)

Healthy At Home Caregivers is growing, and we’re hiring a Care Coordinator to support families, caregivers, and our office team in delivering non-medical in-home care.
What You’ll Do
Care Coordination & Assessments
- Manage referrals and coordinate client assessments
- Schedule and complete limited in-person assessments (designated field day — typically Wednesdays)
- Communicate clearly and compassionately with families and caregivers
- Ensure assessment documentation is accurate, complete, and submitted on time
- Coordinate clean handoffs to scheduling so care starts smoothly
Service Tracking & Follow-Up
- Track service starting dates, renewals, and approved hours
- Follow up proactively so no services or hours are delayed or missed
- Support alignment between care delivery, documentation, and office workflows
Administrative & Office Support
- Maintain accurate client and caregiver records
- Handle documentation, forms, and follow-ups with attention to detail
- Support office workflows during busy periods
- Ensure actions are documented and easy for others to follow
Team & Growth Support
- Support leadership with coordination and documentation
- Identify workflow gaps and improvement opportunities
- Be a reliable, calm presence in a fast-moving office

- Approved hours are not lost due to missed follow-ups
- The office runs smoothly — without constant fire drills
Requirements
Requirements & Compliance
- Ability to pass a Live Scan (background check)
- Ability to complete a TB screening
- Valid driver’s license required
- Comfortable working in the office and attending limited field visits
- Use of company vehicle during work assignments
