About the Role

Position Summary

The ICF Case Manager is responsible for coordinating and overseeing services for individuals residing in an Intermediate Care Facility (ICF) setting. This role ensures compliance with all federal, state, and agency regulations while supporting person-centered planning, health, safety, and quality of life for individuals with developmental disabilities. 


This is an on-site position at Parc Center for Disabilities, working as part of a collaborative, mission-driven team.


 

Key Responsibilities

  • Coordinate and monitor all aspects of care for individuals in the ICF program.
  • Develop and maintain Individualized Service Plans (ISPs) in compliance with ICF/DD regulations.
  • Ensure compliance with AHCA and APD standards.
  • Conduct documentation audits for accuracy and completeness.
  • Monitor medical, behavioral, and overall health needs.
  • Collaborate with interdisciplinary teams, families, and providers.
  • Participate in care plan and team meetings.
  • Complete required reports and documentation in a timely manner.
  • Monitor incident reports and ensure follow-up actions are completed.
  • Train and support direct care staff on plans and compliance requirements.
  • Assist with surveys, inspections, and quality assurance initiatives.
  • Maintain strict confidentiality

Requirements

 

Qualifications

  • High School Diploma or equivalent required.
  • Associate or Bachelor’s degree in Social Work, Human Services, Psychology, or related field preferred.
  • Minimum of 2 years’ experience working with individuals with developmental disabilities required.
  • ICF/DD or case management experience strongly preferred.
  • Valid Florida Driver’s License.

Required Skills & Competencies

  • Knowledge of ICF/DD regulations and compliance standards.
  • Strong organizational and documentation skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Effective communication and teamwork skills.
  • Proficiency in Microsoft Office and electronic health records.
  • Commitment to person-centered care.

Typical hours: Monday through Friday, 7:30 AM – 3:30 PM.

  • This is an on-site position at Parc Center for Disabilities.
