About the Role
<p>We are currently hiring<strong> Clinical</strong>&nbsp;<strong>Care Coordinators</strong> for our Amber House CSU/CRT, a 10-bed voluntary-only CSU whose purpose will be to assess individuals who are having a mental health crisis and are in need of assessment, stabilization, and brief treatment. &nbsp;The service is available to individuals for up to 24-hours. The Amber House CRT has up to 14-beds for individuals in crisis who do not meet medical necessity criteria for hospitalization and would benefit from treatment and supportive programming.&nbsp;</p><p style="font-weight: 400;"><strong>Open Positions:</strong></p><ul><li><strong>Swing 4pm to 12:30 am |Thurs - Monday</strong></li><li><strong>On-Call positions&nbsp;</strong></li></ul><p><strong>CRISIS CLINICIAN RESPONSIBILITIES:</strong></p><ul><li>Delivery of individual, family, group counseling, individual rehabilitation, and case management/brokerage for individuals within the caseload as assigned.</li><li>Utilize Evidence-Based Practice for the designated population/program to assure high-quality results and intended impact.</li><li>Assess an individual&rsquo;s risk status including risk-based symptoms, substance use, environment, physical conditions, etc, and intervene appropriately.</li><li>Creating a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.</li><li>Outreach and progressive engagement and reporting disengagement to the team immediately. Responsible for working with the outreach team and determining when to disenroll individuals based on program protocols.</li><li>Assessment, screening referrals, service planning, community planning, and benefits coordination support wellness and recovery principles.</li><li>Documents and maintains all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies.</li><li>Participates in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by the manager.</li></ul><p><strong>QUALIFICATIONS:</strong></p><ul><li>ASW, AMFT, or APCC with current BBS registration; and 1 year of social service experience</li><li>LCSW, LMFT, or LPCC with current BBS registration; and 1 year of social service experience</li><li>LVN or RN with current CA license and 1 year of social service/psychiatric experience&nbsp;</li></ul><p><strong>Benefits/Perks</strong></p><ul><li>Highly competitive compensation $45/hr (equivalent to $93,500 annually) ASW or AMFT, $104,000 exmpt for LCSW/LMFT.&nbsp;</li><li>+3% wage differential for tested competency in threshold languages for Alameda county (Chinese, Cantonese, Spanish, Vietnamese, Mandarin, Farsi, Cambodian and Tagalog)</li><li>Fully paid medical, dental, vision, and life insurance coverage for employees &amp; dependents &ndash; nothing comes out of your check.</li><li>31 days PTO&nbsp;</li><li>Clinical supervision hours towards licensure</li><li>403b plan.&nbsp;</li><li>Free iPhone with unlimited data for personal/professional use + laptop for easy mobility*</li></ul><p>&nbsp; &nbsp; &nbsp; &nbsp;* Most direct service positions</p><p><strong>ABOUT BACS</strong></p><p>Bay Area Community Services is a CARF accredited non-profit, community-based agency celebrating 70 years of serving Alameda, Contra Costa, Sacramentoand Solano Counties by providing mental health and social services.
