About the Role

Medical Case Manager( Stamford, CT)

Overview

The Medical Case Manager is responsible for helping clients living with HIV/AIDS access and remain in care, including mental health, oral health and support services.


Supervision: Reports to Senior Case Manager


Duties and Responsibilities:

  • To meet with clients to assess and prioritize their needs
  • To fill out paperwork and applications on a continual basis for entitlements, emergency care funds and more.
  • To maintain updated client files including biannual assessments, service plans and monthly progress notes.
  • To advocate for clients when they are not able to advocate for themselves
  • To make home visits, accompany clients to medical visits and participate in outreach efforts when needed
  • To make referrals to other agencies and to follow-up on those connections
  • To collaborate with the other Medical Case Managers about new cases
  • To work closely with the entire HIV Care Team and other community agencies
  • To attend the Immunology Clinic twice monthly as necessary
  • To attend and participate in Bi-weekly staff meetings, monthly Case Reviews
  • To have bi-monthly supervision sessions with Supervisor
  • To attend 20 hours of continuing education per year.
  • Other duties as assigned


Performance Requirements:

  • Must be able to serve as a positive representative of Family Centers CARES
  • Must be sensitive to culture, race, sexual orientation, age, gender, etc.
  • Must be trustworthy and able to uphold strict confidentiality policies
  • Must have experience in intensive case management
  • Experience/knowledge in HIV/AIDS is a plus


Qualifications:

  • Be able to take individual initiative, but also be a team player
  • Determined
  • Bi-Lingual (Creole or other) preferred
  • Able to take individual initiative, but also able to be a team player
  • Comfortable interacting with individuals and groups of people
  • Must have own vehicle and have valid driver’s license
  • BS/BA/BSW or case management experience
