We’re seeking a compassionate, organized, and mission-driven Qualified Intellectual Disabilities Professional Case Manager to support adults with developmental disabilities in living full, meaningful lives.

What You’ll Do:

  • Build relationships with clients and families, ensuring services reflect each person’s unique needs and wishes.
  • Coordinate service agreements, authorizations, and referrals.
  • Develop and implement strategies in collaboration with families and support teams.
  • Maintain accurate electronic records (Quantum) and case notes.
  • Provide advocacy and support during crises.
  • Partner with community resources and internal teams to ensure holistic care.
  • Travel throughout Cook County for client visits (about 70% in the community).

What We’re Looking For:


Must be QIDP-eligible upon hire which includes:

  • At least 1 year working with adults with developmental disabilities.
  • Bachelor's degree in Human services field, including, but not limited to Sociology, Special Education, Rehabilitation Counseling or Psychology, Recreation or in a specialty area such as Art, Dance, Music or Physical Therapies.

Also required:

  • Valid driver’s license and reliable vehicle.
  • Flexibility and willingness to travel locally (mileage and gas reimbursed).
