Symphony Applewood is seeking a compassionate and dedicated Social Service Director to support our short-term rehab and long-term care guests. This role is responsible for providing psychosocial support, discharge planning, advocacy, and care coordination while working closely with residents, families, and the interdisciplinary team.

Responsibilities
- Conduct psychosocial and cognitive assessments
- Develop and maintain resident care plans
- Assist with discharge planning and transitions of care
- Provide emotional support and advocacy for residents and families
- Collaborate with nursing, physicians, and interdisciplinary team members
- Maintain accurate documentation and participate in care conferences
- Support end-of-life, hospice, and palliative care discussions

Requirements
Qualifications & Experience
- Bachelor’s Degree required
- Minimum 2–3 years of experience in long-term care and short-term rehabilitation preferred
- Strong conflict resolution and problem-solving skills
- Excellent communication and interpersonal abilities
- Knowledge of federal and state regulations related to long-term care
- Strong organizational and time management skills
- Compassionate, professional, and empathetic demeanor
- Knowledge of PointClickCare preferred
