About the Role

Description

BMS Family Health and Wellness Centers is seeking a dedicated and compassionate Psychiatric Nurse Practitioner (PMHNP) to join our integrated care team. The position is part-time, 21 hours per week, and will provide comprehensive mental health services to support both pediatric and adult populations in a collaborative, patient-centered environment.


The Psychiatric Nurse Practitioner will play a key role in delivering high-quality care, including psychiatric evaluation, diagnosis, treatment, and ongoing management of acute and chronic mental health conditions. 


This position offers the opportunity to work within a multidisciplinary team committed to improving access to behavioral health services in a primary care setting.

Key Responsibilities

  • Conduct comprehensive psychiatric evaluations, risk assessments, and diagnostic formulations 
  • Develop and implement individualized treatment plans, including medication management and therapeutic interventions 
  • Provide ongoing care for patients with acute and chronic mental health conditions 
  • Collaborate closely with physicians, social workers, nurses, and other care team members to ensure coordinated, holistic care 
  • Participate in care coordination with internal and external providers as needed 
  • Engage in preventive care activities, including mental health screenings and early intervention strategies 
  • Maintain accurate, timely, and compliant documentation in the Electronic Health Record (EHR) 
  • Participate in regular chart reviews and quality improvement initiatives 
  • Contribute to monthly reporting, including productivity and clinical outcome measures 
  • Support and adhere to organizational policies, procedures, and clinical protocols 
  • Participate in the on-call schedule as required 
  • Collaborate with leadership, including the Assistant Medical Director and Nurse Manager, to support clinical excellence and regulatory compliance 
  • Ensure compliance with Managed Care, SBHC contracts, and Federal and New York State regulations

Requirements

Qualifications

  • Active and unrestricted New York State license as a Nurse Practitioner 
  • Board Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) 
  • Minimum of two (2) years of clinical experience as a Nurse Practitioner, preferably in behavioral health or integrated care settings 
  • Strong clinical assessment, diagnostic, and treatment planning skills 
  • Excellent communication and collaboration skills within interdisciplinary teams 
  • Commitment to delivering compassionate, culturally competent, and patient-centered care 
  • Experience with Electronic Health Records and clinical documentation standards
