Over 47 per cent of the more than 4,050 Sḵwx̱wú7mesh Úxwumixw Members live on-reserve, and membership is determined by guidelines set out in the Squamish Nation Membership Code. The Nation has never ceded or surrendered title to its lands, rights to its resources or the power to make decisions within its territory.

Are you a compassionate and skilled nursing professional with a passion for supporting holistic, community-based care? Squamish Nation is seeking a Home Care Nurse Assessor to join the Yúustway Health and Wellness Department within the Nexwnínlhewá7nem Ch'áwch’aw People Services Division.

This role plays a vital part in supporting the health, independence, and wellbeing of community members by coordinating and assessing home care services. More than a clinical role, this is an opportunity to deliver culturally safe care and build meaningful relationships with clients, families, and community partners.

What You’ll Do

  • Develop and implement client-centered care plans with input from families and community supports.
  • Coordinate home care services in collaboration with the Home and Community Care team and allied health professionals.
  • Conduct client assessments and maintain accurate, confidential health records.
  • Provide clinical guidance and supervision to Health Care Assistants and Homemakers.
  • Build and maintain strong relationships with internal teams, external providers, and community partners.
  • Advocate for clients and support access to appropriate health services and referrals.
  • Participate in program planning, development, and evaluation activities.
  • Promote culturally safe, respectful, and holistic care practices.
  • Participate on committees and contribute to continuous improvement initiatives.
  • Perform additional duties as required to support departmental goals.

What We’re Looking For

  • Bachelor of Science in Nursing (BSN) preferred; LPN with relevant experience may be considered.
  • Current registration and good standing with the BC College of Registered Nurses.
  • Minimum 3 years of relevant nursing experience in community or public health settings.
  • Strong understanding of client assessment, care planning, and case coordination.
  • Knowledge of EMR systems and intermediate proficiency with Microsoft Office.
  • Indigenous Cultural Safety training (or willingness to complete).
  • Excellent interpersonal, communication, and relationship-building skills.
  • Valid First Aid and CPR certification (or willingness to obtain).
  • Valid BC Driver’s License and access to a reliable vehicle.
  • Ability to travel to Squamish Valley and work flexible hours as required.
  • Successful completion of a Criminal Record Check (Vulnerable Sector).
  • Experience working in a First Nations community is an asset.

Salary: $80,026 - $120,040

Candidates are typically offered compensation between the beginning to the midpoint of the range. We are committed to providing a fair and equitable package that reflects the value an individual brings to the organization.
