Float Specialty Nurse Case Manager (HIV/Aids Ryan White Program, LVN or RN)

Description

We are seeking a compassionate, organized, and patient-focused Float Specialty Nurse Case Manager to provide advocacy, care coordination, and guidance for patients with complex medical and social needs at our Dora Street and Lakeview Health Centers. Working collaboratively with multidisciplinary care teams, the Nurse Case Manager ensures patients receive high-quality, comprehensive care while empowering them to make informed healthcare decisions and improve their overall quality of life.

Key Responsibilities

• Manage assigned patient caseload and coordinate care plans
• Conduct patient assessments, education, and action planning
• Support treatment adherence, medication reconciliation, and follow-up care
• Coordinate referrals to community resources and specialty services to address medical, behavioral, social, and financial barriers
• Monitor labs, provider visits, and medication adherence

Requirements

• Active California LVN or RN license
• Completion of an approved IV therapy course
• Current BLS (Basic Life Support) certification
• Willing and able to travel between sites 
• Working knowledge of Electronic Medical Records use and process

Preferred Qualifications 

• Bilingual, English/Spanish, fluent 
• Previous experience working as an LVN or RN in an outpatient setting
