The Rehabilitation & Wellness Pavilion features 20 private rooms for short-term rehabilitation and is also home to a membership-based health club for seniors over the age of 55. The Wellness Center boasts the latest fitness equipment as well as a warm, saltwater pool for exercise and therapy. The more than 30 physical, occupational, and speech therapists are part of the skilled nursing and medical team providing inpatient and outpatient services to residents and guests from the community. Skilled nursing is provided in a household model of care led by a household coordinator and a dedicated team skilled in meeting the personal care needs of each resident. The Davis Community also offers home care services in select Porters Neck, Landfall, and other defined neighborhood communities. About the Role

We are currently seeking a full time Nurse to work in our Skilled Nursing Facility. We work 12hour shifts &7a-7p/7p-7A)

MAJOR WORK ACTIVITIES:

  1. Collect data with regard to resident’s physical and mental health including reaction to illness and treatment regimens.
  2. Record and report results of nursing assessment to physician, other personnel and responsible party as appropriate.
  3. Deliver and record licensed nursing care to include medications and treatments as prescribed by the physician as well as care outlined in the resident’s individual plan of care (ADL’s, nutrition, hydration, labs retrieval/oversight, weight oversight, therapies and restorative initiatives, splinting, assistive devices, skin audits, behavior logs, acute episodic oversight documentation, etc.).
  4. Coordinates and provides oversight to non-licensed personnel in implementing the resident’s individual plan of care.
  5. Ensure continuity of care through documentation and communication of resident status (report) to relieving personnel.
  6. Complete all clinical chart documentation according to best practice at time service rendered, prior to end of current shift, and/or by “late entry” with date, time, and legible signature or initials where appropriate. Documentation that is not time specific/shift specific, i.e. such as MDS, Care Plans, and certain summaries may be completed with signature and date on the day of documentation, completion and/or at time of review.
  7. Ensures care plan/care cards are audited to reflect current baseline functionality/interventions.
  8. Respond promptly to resident needs.
  9. Maintain safe and sanitary work environment.
  10. Participate in Quality of Care and Quality Improvement activities.
  11. Maintenance of nursing credentials to include CPR.
  12. Perform specific work duties and responsibilities as assigned by the shift supervisor, Nursing Coordinator, Assistant Director of Nursing and/or the Director of Nursing.

Requirements

Education: High School Diploma or equivalent with graduation from an accredited school of nursing


Licensure/ Certification: Current North Carolina licensure as a Licensed Practical Nurse or Registered Nurse


Experience: Some previous geriatric nursing experience is preferred. An equivalent combination of education and experience may be considered.
