About the Role

Description

At Venture Forthe, we believe that exceptional care starts with exceptional support. We are seeking a compassionate, skilled, and engaging Licensed Practical Nurse (LPN) to join our team as an LPN – Mentor & Training Support professional. In this role, you will help mentor and guide Personal Care Aides (PCAs), Home Health Aides (HHAs), and Certified Nursing Assistants (CNAs) as they begin and grow their careers in home care.

• Support and mentor new caregivers during their first 60–90 days of employment

• Help reinforce classroom training through real-world coaching and support

• Partner with RN Trainers during classroom instruction and skills labs

What You’ll Do

• Conduct regular mentorship check-ins with newly hired caregivers

• Provide coaching on communication, professionalism, documentation, and client interactions

• Reinforce safe care practices, infection control, and body mechanics

• Travel to training locations or client homes for observational support and mentorship

• Assist RN Trainers with classroom instruction, skills labs, and demonstrations

• Support learners who may require additional coaching or remediation

• Assist HR and Recruiting with prep days and new hire onboarding paperwork. 

• Collaborate with HR, Recruiting, and leadership teams on workforce development initiatives

• Identify and escalate clinical or safety concerns appropriately

Requirements

What We’re Looking For

• Current NYS Licensed Practical Nurse (LPN) license and registration

• Minimum of 2 years of experience in home care, long-term care, or community-based settings preferred

• Strong communication, coaching, and relationship-building skills

• Passion for teaching, mentoring, and supporting frontline caregivers

• Ability to provide constructive feedback with empathy and professionalism

• Comfort working independently and collaboratively across departments
