The care provided to patients and families is carried out by Snowline's Interdisciplinary team of professionals, including physicians, nurse practitioners, registered nurses, licensed vocational nurses, medical social workers, spiritual care providers, hospice aides, and volunteers. Snowline's dedicated staff are experts in delivering end-of-life care, with many earning advanced certifications from the Hospice and Palliative Credentialing Center and the Hospice and Palliative Nurses Association. Snowline is a proud Employer Partner with the Hospice and Palliative Nurses Association and encourages our team to seek advancement and certification. Hospice benefits are included in Medicare and Medi-Cal and most health insurance plans. The uninsured and palliative care recipients are also served at Snowline through funds from the proceeds of Snowline's thrift stores, a recycle and repurpose processing center enterprise, individual donations, fundraisers, and the United Way. At Snowline, neither patient nor family will ever receive a bill for services. We serve the western slope of El Dorado, Placer, and Sacramento County, including Sacramento, Folsom, Fair Oaks, Elk Grove, Roseville, Granite Bay, Fair Oaks, Carmichael, Lincoln, Rancho Cordova, and Rancho Murieta.
About the Role

Snowline Health is seeking an experienced, compassionate Social Services Director to lead our social services programs and support our interdisciplinary hospice care team. This leadership role offers the opportunity to guide program development, support staff, and ensure exceptional psychosocial care for patients and families navigating serious illness, grief, and end-of-life care.

Location: El Dorado County / Greater Sacramento Region


Employment Type: Full-Time


Schedule: Monday–Friday (typically 8:00 AM – 5:00 PM; flexibility required), On Call Rotation Required

Position Summary

The Social Services Director provides leadership, oversight, and strategic direction for Snowline Health’s social services programs, including hospice and bereavement services. This role ensures delivery of high-quality psychosocial care that aligns with patient goals, promotes dignity, and supports families throughout the care continuum.


Responsibilities include:

- Provide leadership and oversight of social services programs and staff

- Supervise, mentor, and support social workers and related team members

- Ensure delivery of compassionate, high-quality psychosocial care

- Lead program development and continuous improvement initiatives

- Collaborate with interdisciplinary leaders to optimize patient outcomes

- Ensure compliance with regulatory and accreditation standards

- Oversee documentation quality, productivity, and workflow efficiency

- Support staff engagement, training, and professional development

- Partner with community organizations and external agencies

- Assist with complex patient, family, and staff situations

- Promote a culture of compassion, accountability, and excellence


Salary Description

Competitive, market-aligned salary based on experience and internal equity

Requirements

- Master of Social Work (MSW) required

- Licensed Clinical Social Worker (LCSW) required

- Advanced certification (e.g., ACHP-SW) preferred

- Minimum of 5 years of clinical experience in healthcare, hospice, or related field

- Minimum of 3 years of leadership or supervisory experience

- Experience with program development preferred

- Experience supporting patients and families with grief, loss, and end-of-life care

- Strong leadership, communication, and interpersonal skills

- Ability to work effectively with diverse populations
