Bergen’s Promise is the designated Care Management Organization (CMO) for Bergen County, New Jersey. We provide care management services and coordinate an array of community based resources for youth ages 5-21 and their families. The youth we serve are challenged by serious behavioral, emotional, mental health, substance abuse issues and developmental disabilities. Bergen’s Promise uses a Wraparound Model of Care, and provides a fresh perspective on helping families deal with complicated emotional and behavioral issues. Since 2002, Bergen’s Promise has worked with over 4,000 youth and their families by helping them receive the necessary services and supports to achieve their goals. About the Role

Description

Position Overview:

The Director of Care Management oversees all aspects of program development, implementation, and monitoring ensuring compliance with business, state, and federal guidelines. This role involves recommending, developing, and executing initiatives while evaluating outcomes to enhance care management performance. The Program Director collaborates closely with provider partners and stakeholders to drive continuous quality improvement. 


Responsibilities:

  • Strategic Planning: Developing program goals, objectives, and strategies aligned with organizational priorities. 
  • Program Design and Development: Creating program curriculum, structure, and delivery methods. 
  • Staff Management: Hiring, training, and supervising program staff.
  • Budget Oversight: Managing program finances, including allocating resources and monitoring expenditures.
  • Implementation and Execution: Overseeing the delivery of program activities and ensuring adherence to timelines.
  • Performance Evaluation: Monitoring program effectives through data collection and analysis, making necessary adjustments. 
  • Stakeholder Engagement: Building relationships with key stakeholders, including funders, community partners, and program participants. 
  • Reporting and Communication: Providing regular updates on program progress to leadership and stakeholders. 
  • Assist COO in supervising business functions as it relates to service delivery. 
  • Create relationship driven people initiatives. 
  • Build trust and partnerships with interdepartmental subject matter experts. 
  • Lead and manage the quality of in-person care management.
  • Works as a liaison with external organizations to ensure lines of communication are in place for member engagement.
  • Responsible for monitoring all work product to ensure it is in compliance with all State and/or Federal mandates. 
  • Evaluates departmental operations with a focus on process improvement. 
  • Continuously improving effectiveness and efficiency of processes and program. 
  • Aligning services and care delivery to the strategic plan. 
  • Additional duties as assigned.


Measurable Deliverables:

  • 95% compliance with relevant department audits, department quality improvement plans, CARF measures and in person visit requirement. 
  • 75% or higher retention rate
  • 95% compliance with reporting and documentation benchmarks
  • 100% compliance with agency zero-tolerance policies, and mandated reporting and training
  • Successful completion of quarterly timeline/outcomes outlined on tactical strategic plan
  • Meets annually set strategic benchmarks.

Requirements

Qualifications:

  • Master’s Degree in a related field (such as Social Work, Counseling, Clinical Psychology) 
  • Minimum of three years post-master’s related supervisory experience supporting children with mental health needs  
  • Experience and/or Certification in Developmental Disabilities and/or Drug and Alcohol field of discipline a plus 
  • Experience with Community relations, resource development, and Wraparound process a plus 
  •  Clinically and culturally competent/responsive, with the training and experience necessary to manage staff and complex family needs across child-serving systems 
  •  Valid and Current Driver’s License 
  • Maintenance of license in appropriate discipline (LAC/LPC or LSW/LCSW)
