Description

Status: Full-Time, Non-Exempt. This position is paid on an hourly basis and is eligible for overtime pay in accordance with Colorado state law.

Salary Range: $27.50/hour

Location: Denver, CO

Position Reports to: Nest56 Program Manager 

Supervises: N/A

SUMMARY OF POSITION 

The Case Manager helps tenants of 2700 Wewatta identify and achieve goals that help them maintain safe, stable housing, employment, and education. This person guides tenants toward empowerment, community connectedness, self-sufficiency, and independence. The role may also include managing the front desk part-time. This will entail ensuring secure access to the building, meeting and building rapport with tenants, and assessing and intervening in crisis or safety incidents.


ABOUT Nest56: 

Nest56 is a 56-unit supportive housing complex for young people between the ages of 18-24 in which TGTHR is the contracted service provider. While living here, residents are offered employment and education assistance, support and inclusivity groups, counseling services, life-skills development, mental and physical wellness support, housing navigation when they leave, and an array of individual and group activities.


PRIMARY RESPONSIBILITIES


Direct Case Management

  • Manage a dedicated caseload of up to 28 tenants, building trusting relationships and creating an environment that fosters independent living and community building.
  • Assess tenants' needs, develop and document individualized case plans, make appropriate referrals, and ensure consistent follow-through.
  • Provide tailored support to tenants based on issues identified in the OneHome/HMIS Assessment, TGTHR’s Psychosocial assessment, and periodic life skills assessments.
  • Monitor progress with voluntary participants on a monthly and quarterly basis, tracking tenant progress toward self-sufficiency via surveys, assessments, and other data collection methods.
  • Drive and accompany tenants to various appointments as needed.

Housing Transition & Life Skills Support

  • Assist tenants with basic needs (e.g., laundry, shopping), personal finance management, pro-social skills, and essential life skills necessary to maintain stable housing.
  • Facilitate resident groups focused on developing life skills and fostering community engagement.
  • Partner with tenants to develop a housing transition plan out of 2700 Wewatta and into self-sufficient housing.
  • Advocate to leasing agencies on behalf of tenants receiving Housing Vouchers or other forms of housing assistance.

Administrative & Compliance

  • Complete all documentation accurately and timely using the appropriate data entry systems, including Salesforce, HMIS, and paper files.
  • Be available for team and tenant meetings as needed and as often as requested.
  • Perform other duties as assigned by the Housing Programs Manager.

Requirements

POSITION QUALIFICATIONS


Education & Experience

  • Education: Bachelor’s degree in social services or a related field
  • Experience: 2–4 years working with vulnerable, at-risk, and/or unhoused young people in shelter, transitional, or housing programs.

Note: An equivalent combination of education and experience will be considered in lieu of a degree

Technical & Administrative Skills

  • Software: Proficient in G-Suite and Microsoft Office; experience with Salesforce preferred.
  • Local Knowledge: Knowledge of community resources in the Denver area, with a preference for familiarity with HUD and City of Denver housing regulations.
  • Language: Spanish language proficiency is highly preferred.
  • Logistics: Valid driver’s license and access to reliable transportation.

Physical & Environmental Requirements

  • Ability to sit for extended periods while performing typing or administrative tasks.
  • Ability to lift a minimum of 30 lbs, bend, stoop, kneel, crouch, push, and participate in strenuous activities such as cleaning.

COMPENSATION AND BENEFITS


Salary:  $27.50/hour
