About the Role
Description
What We Offer:
Starting Pay: $23.00-$25.00 per hour, with an increase to $24.00-$26.00 per hour after completing our Train the Trainer series
- Paid training including a comprehensive introduction to working with individuals with intellectual and developmental disabilities, and Mandt certification

Your Role:
- Coordinate medical appointments, referrals, and follow-up care
- Attend appointments and help track provider recommendations
- Communicate with providers, nursing staff, guardians, and residential teams
- Maintain accurate medical documentation and tracking systems
- Help ensure continuity of care for medically complex individuals

Requirements
What We're Looking For:
- Experience in healthcare, medical offices, care coordination, residential services, or a related field
- CNA experience and / or Medical Assistant credential preferred
- Strong organizational and communication skills
- A proactive problem solver who can manage multiple priorities and follow tasks through to completion
- Valid driver's license with at least 5 years of driving experience and an acceptable driving record
- Ability to pass required background checks
IDD experience is preferred but not required.
