Social Services Manager Responsibilities

The Social Services Manager will oversee and coordinate social services for residents within our Skilled Nursing and Long-Term Care facility.

Duties Include:

  • Assess residents’ psychosocial, emotional, and discharge planning needs
  • Develop and implement individualized social service interventions
  • Coordinate discharge planning and transitional care services
  • Assist residents and families with care planning and community resources
  • Facilitate advance directive discussions and decision-making
  • Coordinate Family Council meetings and family support initiatives
  • Manage resident grievance and complaint processes
  • Ensure compliance with state, federal, and company regulations
  • Collaborate with interdisciplinary healthcare teams to enhance resident quality of life
  • Advocate for resident rights, dignity, and emotional well-being

Qualifications for Social Services Manager

Required Qualifications:

  • Bachelor’s Degree in:
    • Social Work (BSW)
    • Human Services
    • Psychology
    • Sociology
    • Gerontology
    • Rehabilitation Counseling
    • Special Education or related field
  • Minimum one (1) year of supervised social services experience in a healthcare setting
  • Experience working with elderly residents or geriatric populations
  • Strong communication, documentation, and organizational skills

Preferred Qualifications:

  • Licensed Social Worker preferred (LBSW, LSW, LMSW, LCSW, etc.)
  • Prior Skilled Nursing Facility (SNF), Rehabilitation, Long-Term Care, Nursing Home, or Healthcare Social Services experience strongly preferred

Qualifications

ENTRY QUALIFICATIONS· An individual with a minimum of a bachelor’s degree in social work or a bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology and· One year supervised social work experience working directly with individuals in a health care setting.· Current Social Work license in the state, if applicable.
