Under the direct supervision of the Director of Women's Services/Nurse Manager, the HUC/Birth Certificate Clerk is responsible for obtaining the necessary information to complete the Legal Birth Certificate, preparing and processing the birth data for entry into the Department of Vital Statistics state computer system, completing the Complimentary Birth Certificate for the parents. He/She also files new birth information with the State of Washington when a previous birth has been misfiled. He/She maintains a daily birth log of all deliveries, correct infants name and statistics so Medical Records may update Providence Sacred Heart Medical Center Meditech computer system with current information. He/She as a Registered Notary Public also completes Paternity Affidavits when necessary for expectant or delivered parents, and complies the billing list of affidavits for reimbursement of State funds.

Required Qualifications:
- General office and/or former employer(s) experience
Preferred Qualifications:
- Medical Terminology, Business, and/or Secretarial course/training
