Health Affiliates Maine is seeking Veterans Case Managers to provide per diem mental health case manager services to Maine Veterans within the Androscoggin County as well as throughout the state of Maine building a caseload in the geographic location and via telehealth.
$500-1,000.00 sign-on bonus. Paid after successful completion of 90 days of employment. (Stipulations apply)
Position Summary:
Case Managers will work collaboratively with Veterans to access the resources and supports available to foster mental health, recovery and wellbeing, reducing the risk of suicide amongst Veterans.
Case Management Requirements:
- Complete CM Program Orientation.
- Provide community-based case management services.
- Maintain a caseload size of 10-50 clients. 10 client minimum to be met within 90 days of employment.
- Provide CM services that follow all applicable agency and program policies and procedures.
- Understand the CURES act and ensure that documents are shared with youth/guardians according to CURES policies and procedures.
- Understand client confidentiality and maintain the confidentiality of client records and information, following HIPAA and 42CFR.
- Have knowledge of the MaineCare and Licensing regulations and standards.
- Demonstrate strong collaboration with service providers involved with client care through regular contact around provision of services.
- Be responsive to requests from the billing, utilization review and administrative support staff.
- Submit billing and payroll within set timelines and ensuring all progress notes, additional required forms, and timecard are included and accurate.
- Maintain voicemail and greeting requirements, which includes the ability to leave a message on the voicemail and appropriate instructions for clients in crisis.
- Check and respond to emails and voicemails Monday through Friday within 24 hours.
- Utilize company provided hot spot, phone and laptop for all work tasks.
- Utilize company supplied cell phones for all work related calls.
- Ensure services are provided to clients in a manner consistent with trauma-informed practices, emphasizing strengths, privacy, individuality, choice and cultural differences.
- Provide services that empower clients to utilize and build their strengths, resources and support to meet goals as independently as possible.
- Provide Services in an ethical manner following NASW Code of Ethics and ethical codes pertaining to the role.
- Assess for co-occurring issues of clients over the age of 18 using the AC-OK screening tools, assess readiness to address co-occurring issues based on these assessments, and provide resources to support recovery.
- Provide a comprehensive assessment of the client which includes identification of client needs, including LOCUS assessment and as needs arise during course of services.
- Offer and complete Advanced Directives when appropriate.
- Develop and follow a comprehensive service plan developed with the client/guardian that reflects relevant strengths, resources/support available and resources/supports needed to achieve goals and which establishes timeframes for goal completion.
- Review service plan goals every 90 days or sooner if indicated and clearly document progress towards goal completion, revising and extending timeframes as appropriate.
- Follow all SMI discharge and transfer processes set forth by the Office of Behavioral Health.
- Assist in negotiating referrals and linkages to providers and other services as needed.
- Encourage the health, safety and welfare of client and worker at all times.
- Be the first line of crisis support for clients and maintain a calm, non-defensive, supportive attitude during crisis or potential crisis situations.
- Maintain and complete accurate client files according to agency policies, procedures and in compliance with applicable law, licensing, contracting, Mainecare regulations, consent decree and the rights of recipients
- Complete all required critical and reportable events within timelines set forth by OBH.
- Fully participate in required supervisions, meetings, and trainings by coming prepared, actively engaging and documenting as required.
- Notify and process client information related to client intakes and discharge plans with clinical supervisor.
- Seek supervision appropriately for client concerns, ethical dilemmas, and is responsive to feedback and directive given by Supervisor(s) and Director(s).
- Independently and effectively manage caseload, including the scheduling of client appointments, meetings, trainings, and office time for completion of required paperwork.
- Consistently show effort towards marketing self and case management services to others within their local community in an effort to build their own caseloads, seeking agency assistance and support if needed.
- Participate in mandated reporting per agency policy and Maine Law including the review and submission of critical incidents, reportable events, and DA reports.
- Follow and periodically update a personal plan of self-care to prevent compassion fatigue, vicarious traumatization and maintain ethical and professional conduct & reaching out for supervision when recognizing self-care concerns.
- Perform other duties related to the position as directed by the Management Team.
Veterans/Fox Program Specific:
- Complete Veterans Orientation
- Build rapport with Veterans to provide culturally sensitive case management
- Work collaboratively with the Veterans Fox Team, as applicable
- Attend and participate in internal Veteran meetings
- Attend and participate in internal Fox case reviews and collaboration meetings, as applicable
- Follow all Veteran Grant and Fox Grant policies and procedures, and complete grant specific documentation.
- Participate in external Veteran related events/trainings as requested by supervisor.
- If Fox CM, prioritize taking Fox Referrals and maintain minimum of 10 Fox clients on caseload

Requirements
Qualifications (include but are not limited to):
- High School Diploma or Equivalent and;
- Certified by the Department as a Mental Health Rehabilitation Technician/Community (MHRT/C).
- Associates or Bachelor’s Degree in Social Work, Psychology, Social Services or related field preferred.
- Valid Driver’s license or HIPAA compliant transportation
