About the Role

Description

We are seeking a compassionate and experienced Hospice RN Case Manager to join our dedicated team. In this role, you will provide comprehensive end-of-life care, coordinate patient-centered plans, and support families during a critical time. Responsibilities include managing a caseload of hospice patients, performing clinical assessments, collaborating with interdisciplinary teams, and ensuring compliance with care standards. The ideal candidate is a licensed RN with strong clinical skills, hospice or palliative care experience, and a deep commitment to patient dignity and comfort.


Schedule: Full-time, Weekdays, 8:30 MA - 5:00 PM, Rotating On-Call


Primary Responsibilities and Duties:

  • Conduct comprehensive patient assessments to identify healthcare needs, develop individualized care plans, and update plans as needed.
  • Provide skilled nursing care, including pain and symptom management, medication administration, and patient education, in accordance with the care plan and physician orders.
  • Coordinate and supervise the activities of hospice aides and other healthcare professionals involved in patient care.
  • Communicate effectively with patients, families, physicians, and other members of the healthcare team to update them on patient progress, discuss treatment plans, and address any concerns.
  • Monitor patients' physical, emotional, and spiritual well-being, ensuring their comfort and dignity during end-of-life care.
  • Collaborate with the interdisciplinary team to develop and implement patient care strategies that align with patients' wishes and goals.
  • Ensure compliance with regulatory guidelines, documentation requirements, and facility policies and procedures related to hospice care.
  • Provide bereavement support and resources to patients' families and caregivers as needed.
  • Participate in team meetings to discuss patient care plans, progress, and any barriers to achieving patient goals.
  • Perform all other duties assigned by the Hospice Director or designated supervisor.

Requirements

Job Qualifications:

  • Current Registered Nurse (RN) license in the state of practice.
  • 1 year of experience as an RN in a hospice or related healthcare setting preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and manage time effectively.
  • Valid driver's license and reliable transportation for home visits.
  • Current CPR certification.
  • Knowledge of regulatory requirements and guidelines governing hospice care and documentation.
  • Demonstrated commitment to providing compassionate, patient-centered care in a hospice setting.
  • Experience with electronic medical record systems and documentation preferred.

Physical Requirements:

  • Ability to lift and transfer patients, as well as carry equipment and supplies weighing up to 50 pounds.
  • Capable of prolonged standing, bending, squatting, and walking during home visits.
  • Fine motor skills for documentation and administering treatments.
  • Ability to respond swiftly and efficiently to emergency situations.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
  • Job requires accepting criticism and dealing calmly and effectively with high-stress situations.
  • The ability to use your abdominal and lower back muscles to support part of the body repeatedly or continuously over time without "giving out" or fatiguing.
  • Full range of body motion, including handling and lifting of patients.
  • Manual dexterity, finger dexterity, and eye-hand coordination required.
  • Ability to occasionally lift and carry items weighing 50 pounds or more.
  • Corrected vision and hearing within normal range.
  • Possible stressful working conditions or irregular hours due to the nature of the job and the health stage the patient is in.
  • Exposure to communicable diseases.
  • Must have a current driver’s license and ability to drive to and access patients' homes.
