What You’ll Do

  • Receive and process referrals via phone, fax, EMR, or secure email for hospice, home health, and palliative care.
  • Verify demographics, clinical details, and insurance
  • Handle prior authorizations as needed
  • Explain services and obtain consents when appropriate
  • Ensure timely coordination with the clinical team
  • Keep meticulous referral logs and records
  • Deliver outstanding customer service to patients, families, and partners
  • Join daily stand-ups and contribute to team planning
  • Ensure HIPAA compliance and confidentiality

Requirements

Education & Experience:

  • High school diploma or equivalent required
  • 1+ year in healthcare intake, admissions, or medical office work (hospice/home health preferred)

Skills & Requirements:

  • Must type at least 34 WPM
  • Detail-oriented with strong organizational skills
  • Clear and compassionate communicator
  • Proficient with EMRs and Microsoft Office
  • Insurance and Medicare/Medicaid knowledge a plus
  • Adaptable in a fast-paced, team environment
