About the Role

Description


We're building out a new home healthcare territory and looking for an RN Case Manager to grow with us from the ground up. You'll start by overseeing a single dependent adult case and, as we expand our presence in the area, take on a growing caseload that will include pediatric, dependent adult, and senior patients. This is a chance to help shape how our team delivers care across populations — an ideal role for an RN who wants to be part of something early and build alongside a company that's investing in this market. 


What You'll Do

  • Oversee and coordinate care for patients in the home setting, beginning with one dependent adult case and scaling as the territory grows
  • Take on a growing caseload spanning pediatric, dependent adult, and senior patients as cases come on board
  • Develop, implement, and update individualized care plans in collaboration with physicians, families, and the care team
  • Conduct assessments, monitor patient progress, and adjust plans of care as needs change
  • Serve as the primary clinical point of contact for families, case managers, and referring providers
  • Ensure documentation, compliance, and care delivery meet all regulatory and payer requirements
  • Help establish workflows and best practices as we build out the territory 

Clinical Responsibilities & Required Skills

  • Tracheostomy & ventilator care: Manage trach/vent-dependent patients, including suctioning, trach care and tube changes, ventilator monitoring, and administration of nebulized medications
  • Enteral nutrition & medication: Administer G-tube and J-tube feedings (bolus and continuous) and deliver medications via enteral routes, monitoring tolerance and site care
  • Activities of daily living: Provide and oversee daily ADLs, including bathing, dressing, grooming, and hygiene
  • Mobility & positioning: Perform routine turning and repositioning to maintain skin integrity and prevent pressure injuries
  • LVN supervision: Oversee and support the LVN(s) providing one-to-one care, ensuring care is delivered safely, consistently, and in line with the plan of care
  • Ongoing assessment: Monitor respiratory status, nutritional tolerance, and overall condition, adjusting the plan of care and escalating to the physician as needed

Requirements

  • Active, unrestricted RN license in CA
  • Home health or case management experience
  • Experience with trach/vent, G/J tube, and medically complex patients
  • Comfort working across pediatric, dependent adult, and senior populations as the caseload grows
  • OASIS proficiency required: We complete OASIS documentation across our cases, and this will apply as the caseload grows
  • Strong clinical judgment and the ability to work independently in the field
  • Excellent communication and coordination skills with families and interdisciplinary teams
  • Comfort with an early-stage, growing operation — flexible, proactive, and excited to build
  • Current BLS/CPR certification
  • Reliable transportation and willingness to travel within the territory
