About The Company
Brook is redefining what remote care means — transforming it from device-driven monitoring into holistic, compassionate care that connects patients, providers, and data in real time. Where traditional Remote Patient Monitoring stops at dashboards, Brook partners with health systems, clinics and care organizations to go further — blending advanced AI with experienced clinical care teams to deliver ongoing, person-centered care as a seamless extension of the provider teams. With Brook, results are fast, measurable, and sustainable: • 50% reduction in all-cause readmissions within 30 days • 80% of uncontrolled hypertension patients in control within 6 weeks • 24% A1c reduction in high-risk patients • 66 NPS ('excellent' category) — patients feel supported, not monitored Brook’s full-service partnership model eliminates common barriers to adoption. We deliver the technology, clinical staff, integration, and ongoing management — no IT lift, no added burden on clinicians, and rapid time-to-value. All with no CapEx. Patients receive proactive, personalized support; providers gain relief from growing workload pressures; and health systems see stronger outcomes, higher satisfaction, and new sustainable revenue streams. In a healthcare environment defined by workforce shortages, financial strain, and rising patient expectations, Brook provides what’s missing in most digital health programs: continuity, connection, and compassion at scale. Brook has received multiple industry recognitions for innovation and impact in remote care delivery, including Modern Healthcare’s Best in Business, Stevie Healthcare Technology Company of the Year, Digital Health Awards, Merit Awards for Connected Digital Health, and Pinnacle Awards for Artificial Intelligence and business performance. About the Role

Description

About Brook

Brook is The Remote Care Company. Building on our foundation in remote care, we provide the connected operating system that combines clinical AI, care teams, connected devices, and end-to-end delivery to enable continuous, longitudinal care at scale.

We support adults managing chronic conditions such as diabetes, hypertension, and heart failure, while also helping people reduce their risk of developing type 2 diabetes through Brook+, our CDC-recognized Diabetes Prevention Program.

Working alongside clinics, hospitals, and health plans, we help patients stay connected, engaged, and supported between visits through a combination of technology, clinical expertise, and proactive outreach. The approach enables earlier intervention, improves the patient experience, reduces administrative burden on care teams, and helps healthcare organizations deliver better outcomes more efficiently.

The result is healthier patients, more satisfied providers, fewer avoidable hospitalizations, and a more scalable and sustainable approach to chronic care management.

Our Ambition

Make continuous care the new standard of healthcare.

Run the continuous care infrastructure that extends clinical teams and empowers patients to achieve better health outcomes.

Brook. The Remote Care Company

For our patients. For our partners. For each other.

Job Overview

The Licensed Practical Nurse (LPN), Remote Nurse Monitor serves as the heart of Brook’s care model—combining clinical expertise with compassion to bring human connection to technology-driven care. The LPN acts as an extension of the provider’s office, monitoring patients’ health data in real time and guiding them through daily decisions that support improved health outcomes.

This role builds trusted relationships with patients, helping them interpret their readings, stay engaged in their care plans, and feel supported every step of the way. The LPN collaborates closely with Registered Nurses, Health Coaches, and provider partners to identify early signs of risk, close care gaps, and ensure timely and appropriate interventions.

The position is ideal for a nurse who thrives on meaningful patient interaction, embraces innovation, and seeks to be part of a forward-thinking team redefining chronic care delivery through empathy, data, and technology.

Requirements

Key Responsibilities

Patient Onboarding & Education: Introduces new patients to Brook’s remote monitoring program, explains device setup and usage, and ensures each patient feels comfortable and confident participating in their care.

Health Monitoring & Data Review: Reviews incoming biometric data and patient-reported information to identify changes in condition, trends, or areas requiring follow-up or escalation.

Patient Outreach & Support: Engages directly with patients to provide education, encouragement, and early intervention when health readings fall outside expected ranges.

Care Coordination: Communicates with primary care providers, specialists, and internal Brook team members to share insights, close care gaps, and maintain continuity of care.

Documentation & Compliance: Maintains accurate, timely, and complete documentation in EMR systems; ensures compliance with HIPAA and all applicable privacy and security requirements.

Quality & Process Improvement: Contributes to initiatives that improve patient experience, clinical workflows, and adherence to quality metrics and CPT billing standards.

Collaboration & Partnership: Works closely with the RN Team Lead, Health Coaches, and Clinic Success Managers to ensure seamless communication, consistent patient experience, and operational excellence.

Working at Brook

Three commitments anchor the work — for our patients, for our partners, and for each other. Brook moves fast in two of the most demanding industries in America, and the way the team shows up to each other is the way it shows up to the patients who count on Brook.

  • No bystanders. A gap or a risk gets brought forward, not worked around — effort and credit are shared across the team.
  • Honest, with care. Pushback is thoughtful, and decisions are anchored in what serves patients and partners.
  • Operational rigor, with AI in the loop. Daily, skeptical, accountable use of AI-enabled tooling — and the judgment to know where AI carries the work and where a human must.

Brook is as focused on our employees’ health as we are on that of our patients. Our Benefits program reflects that. We recognize that health does not just mean physical health, but mental and financial health as well. We make every effort to cover all those areas in our plan offerings.

Benefits at Brook

Brook offers a comprehensive benefits package supporting the medical, financial, and mental-health wellbeing of employees and their families.

  • Medical, dental, and vision — Brook pays 100% of premiums for you and your children and 50% for a spouse or domestic partner, plus a Brook-funded HSA contribution (prorated by hire date) and a medical concierge.
  • Mental health at no cost through Spring Health, plus flexible PTO, dedicated sick time, and a generous holiday schedule.
  • Employer-paid life, AD&D, and short- and long-term disability; 401(k) with company match; and an emergency-savings match.
  • Employee referral bonus.

Working Arrangement and Eligibility

This is a fully remote position. Candidates must be authorized to work in the U.S.; the role is not eligible for U.S. visa sponsorship.

Equal Opportunity

Brook Inc is an equal opportunity employer. We are committed to building an inclusive and diverse workforce. Brook does not discriminate on the basis of race, religion, color, sex, gender identity, sexual orientation, marital status, age, non-disqualifying physical or mental disability, national origin or ethnic origin, military service status, citizenship, or any other protected characteristic covered by applicable law.
