Job Description:

  • Part-time or Full-time positions.

  • St Vincent's Care at Home (SVCAH) located in Sydney Private Hospital, Darlinghurst.

  • Minimum 3 years post graduate nursing experience in community, and/or medical-surgical or critical care settings essential.

Medical Care at Home Clinical Nurse

At SVCAH, our innovative Medical Care at Home (MCAH) model replaces hospital beds and diverts patients from Emergency Departments, enabling them to receive high-quality, evidence-based care in the comfort of their own home, workplace, or virtually via telehealth.
This approach not only improves patient outcomes but transforms how healthcare is delivered in the community.

About the Role
As a MCAH Clinical Nurse, you will:

  • Work in a close-knit team alongside the MCAH Leads (Nursing and Medical), Visiting Medical Officers (VMOs), St Vincent’s Care Facilities, and a diverse multidisciplinary team.

  • Assess patient needs, develop tailored care plans, escalate care when required, and coordinate safe discharge.

  • Deliver advanced nursing interventions, including IV therapies, complex wound care, and monitoring of complex conditions.

  • Manage and coordinate new referrals.

  • Promote independence and enhance patient quality of life.

  • Strengthen the connection between hospital and home.

  • Contribute to better health outcomes and a more sustainable healthcare system.

  • Provide guidance and mentorship to new coming staff members to the growing team.

Things that work for us

  • Registered with AHPRA (Australian Health Practitioner Regulation Agency) as a Registered Nurse.

  • At least 3 years post graduate nursing experience in community, and/or medical-surgical or critical care settings essential.

  • Demonstrated interpersonal and communication skills with the ability to build a rapport with others quickly.

  • Availability to work a variety of shifts including weekends, and on call overnight (telephony only).

  • NSW driver's licence with no restrictions.

  • Full and unrestricted working rights in Australia.
