AOD Outreach & Community Engagement Program
Part Time (0.8 FTE) - Fixed Term until 30 June 2027

About the role

North Richmond Community Health (NRCH) is seeking an experienced and compassionate Complex Care Coordinator to join our AOD Outreach & Community Engagement (OAE) Program.

This is an opportunity to work alongside people who use drugs, providing holistic, trauma-informed care coordination and assertive outreach that supports people to achieve their own goals. Working as part of a multidisciplinary team, you'll build trusting relationships, deliver evidence-based harm reduction interventions, and help improve health and wellbeing for some of our community's most marginalised members.

This role is community-based and combines complex care coordination with assertive outreach, clinical assessment, community engagement and harm reduction education throughout the North Richmond precinct.

• Deliver evidence-based, trauma-informed harm reduction services to people who use drugs.

• Undertake assertive outreach and engage with people experiencing complex challenges, including AOD use, homelessness, and physical and mental health concerns.

• Complete holistic assessments, develop care plans and coordinate support within a shared caseload framework.

• Conduct clinical assessments, identifying when escalation to medical services is required and facilitating appropriate referrals.

• Collaborate with the Medically Supervised Injecting Room (MSIR), multidisciplinary colleagues and external services to deliver integrated, person-centred care.

• Build positive relationships with community members and contribute to overdose response, harm reduction education and improved public amenity across North Richmond.

Please note: This position operates across a seven-day roster between 8:30am and 9:30pm. Regular evening and weekend work is an essential requirement of this role.

About you

You are a Registered Nurse who is passionate about harm reduction, social justice and delivering person-centred care. You thrive working autonomously in the community while collaborating closely with a multidisciplinary team and external services.

You will bring:

• Current Division 1 Registration with AHPRA

• Qualifications in Alcohol and Other Drugs (AOD) (or willingness/experience as outlined in the PD)

• Experience supporting people who use drugs and people experiencing complex health and social needs

• Strong clinical assessment and care coordination skills

• A commitment to trauma-informed, culturally safe and person-centred practice

• Excellent communication and relationship-building skills

• Current First Aid Certificate (or willingness to obtain)

Experience in outreach, community engagement, mental health and/or complex case management will be highly regarded.
