Heartline Staffing, a Strategic Talent Partner with Purple Cow Recruiting, is seeking an experienced Registered Nurse – Case Manager for a travel assignment supporting an acute care hospital in Mission Hills, California. This role focuses on inpatient care coordination and discharge planning for medically complex patients in a hospital setting.

POSITION SUMMARY

The Registered Nurse – Case Manager is responsible for coordinating inpatient care, facilitating safe and timely discharges, and collaborating with interdisciplinary teams to ensure continuity of care. This role works closely with physicians, social services, and external providers to manage complex patient needs while adhering to regulatory and payer guidelines.

ASSIGNMENT DETAILS

• Location: Mission Hills, CA  

• Facility Type: Acute Care Hospital  

• Assignment Length: 13 weeks  

• Start Date: 02/09/2026  

• Shift: Days – 5x8s (08:00–16:30)  

• Patient Ratio: Approximately 1:20  

• Floating: Not required  

• Weekend Requirement: Rotating weekends to meet department needs  

• First-Time Travelers: Accepted (clinical screening required)

• Inpatient acute care hospital setting  

• Strong interdisciplinary collaboration with physicians, social services, and transportation teams  

• Epic electronic medical record system

Requirements

MINIMUM REQUIREMENTS

• Active California RN license (must be active by start date)

• Minimum of 2 years of acute care hospital Case Manager experience

• Acute care hospital experience performing care coordination and discharge planning is REQUIRED

• Behavioral health–only case management experience does NOT qualify

• Experience utilizing InterQual criteria

• Experience with inpatient discharge planning in an acute hospital setting

• Epic charting experience REQUIRED

• Willingness to work rotating weekends to meet department needs

• Travel experience required (first-time travelers may be screened)

• Ability to commit to working 3 of the 5 major holidays if required by the client
