About the Role:


As an Infusion and Home Care RN with us, you'll have the opportunity to apply your nursing skills in a dynamic environment. From infusion therapy to wound care, ostomy teaching, medication education, and disease management, you'll play a vital role in enhancing the well-being of our patients. 2 years of home care RN experience is preferred for this position. Work at your own pace, providing quality care and education in the comfort of patients' homes. Must be willing to travel within an assigned area in the valley, but don't worry, you'll be compensated for mileage. 


Pay:


 $80,000 - $100,000 per year

Available Shifts:

 

Enjoy the convenience of working from 3 PM to 11 PM, Full-Time. Full-Time (5 shifts per week) 


Responsibilities:

  • Collaborate with physicians to develop personalized care plans for new patients.
  • Deliver hands-on nursing care and provide patient/family education based on individual care plans.
  • Schedule visit times and communicate effectively with clients
  • Utilize your clinical IV experience to educate patients and caregivers on the safe administration of home IV therapies
  • Provide weekly central line care and lab draws while adhering to infection control practices
  • Coordinate care with supervisor, staffing, and pharmacy staff to ensure patients receive care as prescribed by the physician
  • Proficiency in Microsoft Office and electronic medical records (EMR) programs
  • Complete documentation promptly

Requirements

 Qualifications:

 

  • Proven excellence in customer service with a strong focus on patient satisfaction
  • Positive, proactive attitude and exceptional organizational skills
  • Deep passion for the nursing profession and dedication to quality care
  • Proficiency in Medicare OASIS documentation, infusion therapy, and wound care is a plus
  • Minimum of 2 years of experience in home care nursing preferred
  • Valid Arizona Level One Fingerprint Clearance Card
  • Active Driver's License
  • Current CPR/BLS certification
  • Proof of car insurance
  • Valid TB test results
  • First Aid certification
  • Active RN license in Arizona
