We are seeking a compassionate and dedicated Mental Health Therapist to join our team. The Therapist will provide individual, couples, and family counseling services to clients across the lifespan. This role includes assessment, diagnosis, treatment planning, treatment, crisis intervention, and collaboration with medical and support staff to ensure comprehensive, coordinated care.
Key Responsibilities:
- Conduct biopsychosocial assessments and develop individualized treatment plans
- Provide individual, couples, and family therapy
- Diagnose mental health conditions in accordance with DSM-5-TR criteria
- Maintain timely and accurate clinical documentation in the electronic medical record (EMR)
- Provide crisis intervention and risk assessment as needed
- Collaborate with primary care providers and other team members in an integrated care model
- Participate in case conferences, team meetings, supervision, and peer/chart reviews
- Support care coordination and referrals to community resources
- Comply with all ethical, legal, and regulatory requirements
Qualifications:
- Master's degree in Social Work, Counseling, Psychology, or Marriage and Family Therapy Required
- Current licensure (e.g., LCSW, LMHC, LMFT) or eligibility and active pursuit of licensure, if applicable Required
- Proficient English and Spanish communication skills Required
- Comfortable working with a range of individuals from different ages (children, teenagers, adults), diagnostic conditions, and other diverse populations
- Knowledge of trauma-informed care and evidence-based treatment modalities
- Experience working with individuals experiencing substance use disorders
- Strong documentation and organization skills
