We provide patient care in San Francisco, San Mateo, Santa Clara, Contra Costa, Alameda Counties and Salona County.

What you’ll do as a Licensed Vocational Nurse (LVN) - Hospice:

  • Work under the direction of a RN - Case Manager
  • Provides skilled services to patients in the comfort of their home
  • Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes
  • Respected and treated as a vital member of the patient care team

To be a success as a Licensed Vocational Nurse (LVN) - Hospice:

  • 1+ years Hospice Case Manager Experience required
  • Current and unrestricted CA RN license
  • Current BLS/CPR Certification
  • Experience working with an interdisciplinary team is highly desired
  • Case Management fundamentals is highly desired
