About the Role

 

In this Social Work role, you will be a member of a dynamic team with the opportunity to work in a setting assisting adult patient cohorts from general medicine, general surgery, Emergency department, Women’s and Children’s and other specialty areas as required. The focus is on psychosocial assessments, risk assessments, complex discharge planning and family support.  As a member of the team you are integral to the Monash Health Social Work service delivery and consistently demonstrate our core values. You will be part of a flexible workforce with personalised support. 

 

About Us

 

We are a busy and dynamic team assisting patients and families by providing a professional service for patients referred across the acute and sub-acute inpatient setting. 

 

Our Social Work service includes Acute, Sub-Acute (Aged and Rehabilitation) and specialty areas such as Intensive Care, Emergency Department and Women’s and Children’s services.  

 

The successful applicants will have the opportunity to accept available shifts and some short term contracts across all sites as part of a busy, energetic and friendly Allied Health team. These positions will be based at our hospitals; Casey, Clayton, Dandenong, Kingston, Victorian Heart Hospital and Moorabbin sites.

 

About You 

 

  • Qualified and Experienced: You have a minimum of 4 years post-graduate experience with a strong focus on patient-centered care.
  • Adaptable and Enthusiastic: Your hard-working nature allows you to thrive in changing settings and shifts, including weekdays, weekends, and public holidays.
  • Dynamic Professional: You are passionate about contributing positively to the health and well-being of your patients.
  • Highly Motivated: You strive to perform at your best every day, showcasing excellent communication and interpersonal skills.
  • Committed to Excellence: You are dedicated to safe, best practice and quality care, with an unwavering commitment to delivering patient-centered care.
  • Experienced: Experience in a hospital or other health setting is essential.

 

What you Need

 

To be considered for this important role you will ideally possess the following: 

 

  • A Bachelor’s or entry-level Master’s degree in Social Work, accredited by the AASW, with eligibility for membership to the
  • Australian Association of Social Workers
  • A minimum of four years’ experience in a healthcare setting (preferred)
  • Strong organisational skills and a positive, proactive approach to work
  • Experience supporting individuals from culturally and linguistically diverse backgrounds
  • The ability to work independently and manage competing priorities
  • A friendly, approachable, and dependable nature
  • Advanced clinical assessment skills and expertise in complex discharge planning
  • A current Victorian Driver’s Licence
