About the Role

Case Manager – Mental Health & Justice Case Management Program (Full Time – Permanent)

 



The Role and Opportunity


CMHA Toronto’s Mental Health and Justice Case Management Program is seeking a Site based Case Manager. The Case Manager will work primary at a residential site, within team of case managers primary in the West (sometimes East) end of Toronto to provide residential/community-based support and linkages to individuals with serious mental illness who are at significant risk of incurring criminal charges and/or at significant risk for apprehension by the police.


Salary: $54,943 – $60,203

Hours of Work: 35 per week

Job Description:

As a Case Manager, you will be based at a designated site, playing a crucial role in supporting our residents by developing and implementing individualized care plans. You will work closely with clients to assess their needs, connect them with resources, and provide ongoing support to help them achieve their goals. Your responsibilities will include:


RESPONSIBILITIES

  • Outreach and assessment
  • initial assessments and developing personalized care plans.
  • Providing case management services, including coordinating with external agencies and service providers.
  • Offering emotional support and counseling to residents.
  • Monitoring and documenting residents' progress and adjusting care plans as needed.
  • Facilitating group activities and workshops to promote personal development.
  • Ensuring compliance with all relevant regulations and standards.
  • Collaborating with other team members to provide holistic care.
  • Providing community-based case management supports, including education, resources, accompany to appointments ,information and advocacy
  • Assist with ID cards, OAS, OW/ODSP, Trillium, etc.
  • Linking to community services and agencies and/or providing support and resources
  • Contribute to the development of comprehensive client-centered assessments and recovery plans (OCAN,MSE).
  • Support with medical and or mental health challenges
  • Orientation to program and agency 
  • Client-centred engagement, service provision and interventions
  • Skill development and goal planning 
  • Maintaining a trusting, respectful and collaborative working relationship 
  • Monitoring, evaluation and follow-up
  • Conduct visits in the clients’ homes, hostels, hospitals and other community settings working in sometimes difficult conditions, including hoarding and pest infestations.
  • Comfortable working with a broad range of clientele. 

SKILLS YOU BRING

  • Bachelors level education in Social Work/Social Science or equivalent
  • 3years experience in community mental health and addictions
  • Ability to work with diverse communities
  • Ability to demonstrate a non-homophobic and anti-racism approach
  • Experience in case management
  • Second language would be an asset
  • A valid driver’s license, a car and insurance coverage are required for this position.

Hours of Work:

  • Monday to Friday, 9:00 a.m. to 5:00 p.m.
