At Unity Hospice, we bring peace of mind and comfort to patients and families when it matters most. We’re growing and looking for a PRN Referral & Intake Nurse (LPN) to join our team and serve as a key connection point for patients, families, and referral partners across all Unity territories.


This role is ideal for someone who thrives in a fast-paced environment, enjoys coordination and communication, and is passionate about supporting patients at the very start of their hospice journey.


PRN Requirements: Must work a minimum of 16 hours per month, at least one holiday per year, and weekends as determined by the leader/designee based on departmental need. Typical hours are 8:00 a.m. – 5:00 p.m. 


This role provides coverage across all Unity territories.


What You’ll Do:
As the first point of contact, you’ll play a vital role in ensuring a smooth and compassionate referral process. Responsibilities include:

  • Managing incoming referrals and gathering patient medical information 
  • Communicating with hospitals, clinics, and care teams 
  • Coordinating initial visits with patients, families, and admissions staff 
  • Verifying insurance coverage and obtaining necessary orders 
  • Supporting eligibility review and following up on pending referrals 
  • Maintaining accurate and timely documentation

Requirements


1. Current unrestricted Licensed Practical Nurse licensure from the State of Wisconsin

2. Valid WI Drivers’ License, reliable transportation, and proof of current automobile insurance coverage
