About the Role:
We are seeking an Occupational Therapist (OT) to provide in-home, individualized therapy to adult clients in Grand Junction, CO. You’ll help patients improve independence, mobility, and overall quality of life through skilled, client-centered care.

Compensation & Scheduling:

  • Competitive Pay: $70.00 – $85.00 per routine visit
  • Start of Care Visits: Up to $120 per SOC
  • Part-Time Availability: 10–20 visits per week

Key Responsibilities:

  • Develop and implement personalized treatment plans
  • Conduct evaluations using standardized tools and clinical expertise
  • Provide therapy focused on ADLs, mobility, safety, and independence
  • Educate patients and caregivers on home programs and adaptive equipment
  • Maintain timely and accurate clinical documentation
  • Collaborate with a multidisciplinary care team to ensure coordinated support

  • A valid Occupational Therapist license in Colorado
  • Proven experience in home health or a related field
  • Excellent communication and interpersonal skills
  • Proficiency in electronic medical record documentation
