About the Role
Description
JOB PURPOSE: A nonexempt position responsible for receiving and processing requests for patient records in an accurate, timely and complete manner; responsible for the retention and destruction of electronic and paper medical records in an accurate and complete manner; and for supporting medical recordkeeping and HIPAA compliance in the organization.
JOB FUNCTIONS:
- Perform assigned medical record keeping and maintenance functions; including but not limited to creating and maintaining filing systems and other tools for tracking and processing medical records information; retrieving records and purging files on a regular basis.
- Scanning and Indexing medical records into the Legal Health Record or archiving systems.
- Release of patient medical and billing records as required by patients, healthcare entities, or by law. This includes tracking and processing all requests for records in accordance with company policies and procedures. This position will seek support and clarification from the Compliance Director when indicated. It will also ensure that patients or their designees will receive records in a timely manner upon request.
- Record completion tracking and communication.
- Chart correction and other medical record cleanup.
- Record Integrity/Patient Identity maintenance and cleanup.
- Assists to train and support practice staff in implementing medical recordkeeping best practices and maintain HIPAA and release of information compliance.
- Review Paper Medical Records and determine whether they are ready for destruction as prescribed in the record retention and destruction policy.
- Data Entry of patient demographics from reviewed charts into an inventory and index log.
- Secure records that are ready for destruction in the appropriate shred containers.
- Reorganize Medical Records that are not ready for destruction and pack them back in boxes by destruction year.
- Daily reporting of job progress to the Compliance Director.
- Acts in a manner supporting organizational values and excellent customer service.
- Performs other administrative duties as assigned.
Requirements
Education
- High school diploma or equivalent.
- Completion of Health Information Management, Transcription, or Data Management program preferred.
Experience
- Minimum two (2) years of office or administrative experience.
- Health Information Management/Medical Records experience, with experience in Release of Information, Data Management, or Transcription preferred.
PERFORMANCE REQUIREMENTS:
Knowledge:
- Knowledge of medical terminology.
- Basic knowledge of MS Office, Word and Excel.
- Bilingual English and Spanish, preferred.
Skills & Abilities:
- Ability to prioritize and perform multiple tasks simultaneously.
- Ability to sort and file materials correctly using alphabetic or numeric systems.
- Ability to easily navigate and perform workflows within Electronic Record systems.
- Ability to analyze medical records for completeness and accuracy, paying attention to detail.
- Ability to transcribe data out of the medical record onto patient forms.
- Ability to schedule time and assignments effectively.
Physical Requirements:
- Involves sitting approximately 90 percent of the day, walking or standing the remainder. Some bending, stooping, and lifting up to 15 pounds.
Equipment Operated:
- Standard office equipment including computers, fax machines, copiers, printers, telephones, etc.
Work Environment:
- Position in a well-lit and well-ventilated office environment. Frequent contact with a variety of people.
