Title: IPU Hospice Registered Nurse


Qualifications: 

Registered Nurse from an accredited program, currently licensed and in good standing with the State Board of Nursing in the state of licensure of the hospice program. 

-At least 1 to 2 years of experience in nursing in an acute care setting or 1 year of experience in home health, hospice, or comparable experience is preferred. 

-Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required.

Responsibilities:  


It is the responsibility of the Nurse to coordinate and provide the highest level of hospice care to patients and families. 

This position reports directly to the Director of Patient Care Services.  The Nurse works in conjunction with the Clinical Coordinator and the Interdisciplinary Group to: 


• Assume overall responsibility for organizing patient care from an interdisciplinary group approach. 

• Provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Group members, and other health care professionals involved in the patient’s care. 

• Assess and implement a pain/symptom management program formulated with the participation of family and patient, attending physician, and the Interdisciplinary Group, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. 

• Utilization of the e-prescribe system for ordering of patient medications • assist in facilitating freedom of choice, relief of suffering, and death with dignity for patients facing end-of-life issues. 

• Provide support and education for families based on the hospice philosophy of care.  Care includes physical and psychological. spiritual and financial comfort.  

• Participate in Interdisciplinary Group meetings and update the Plan of Care for assigned patients based on identified problems and needs. 

• Ensure all services provided are in accordance with the patient’s Plan of Care. 

• Ensure the POC involves the active participation of the patient, family, PCP, and the interdisciplinary group. 

• Coordinate all services related to hospice care for assigned patients. Coordination of services includes communicating and making referrals for home health aide/homemaker, spiritual counselor, social work, volunteer, and ancillary services, as well as ordering medications and Durable Medical Equipment. Rev. 08/19-KM 2 

• Educate patient and family on the disease, disease process, and expectations related to the disease. 

• Provide visit coverage throughout the entire service area, as directed.

• Perform assigned visits, including but not limited to: Symptom Management, Wound Care, Imminent Death, Admission, Expiration. 

• Provide availability for the week, by 9 am each Monday morning, to the Director of Patient Care Services. 

• Contact the Director of Patient Care Services at least 2 hours prior to your work time if you will be absent or late.
