About the Role

Description

Easterseals Blake Foundation started as the Cerebral Palsy Foundation of Southern Arizona. Since then, we have expanded our services to better serve our community members. We are now one of Arizona's top providers of comprehensive behavioral health services, child welfare, and services for individuals with intellectual and developmental disabilities. Parents as Teachers is an evidence-based home visiting program that provides parents and caregivers the knowledge, skills, and resources necessary to support the developmental needs of young children.


  General Position Description: Implements the Parents as Teachers (PAT) model by providing parents and caregivers with knowledge, skills, and resources necessary to support the developmental needs of young children.


Essential Duties and Responsibilities:

  • Accepts referrals, then contact families to schedule a home visit to complete intake and enrollment to the Parents as Teachers program within the required timeframe. 
  • Collects child health and developmental information. Determines other screenings which may be required. Assesses family strengths and resources, including informal support systems.
  • Conducts, with family participation, parent skills assessments and caregiver screenings, as well as child developmental, hearing, and vision screenings at required intervals. 
  • Convenes a Family Service Plan meeting to develop functional child and family goals and outcomes to guide supports and services during ongoing service provision.
  • Provides coaching within natural environments to increase family capacity to support their child’s development. 
  • Facilitates referrals to community resources/services. Collaborates with community agencies/service providers to promote timely and seamless services to children and families. Assesses and supports access to emergency needs of families for food, diapers, clothing, and transportation.
  • Plans and provides monthly family/group connection meetings available to families enrolled in Parents as Teachers.
  • Completes all program documentation and data requirements; maintain confidential, individual records. Supports families through transitions to other services as necessary. 
  • Completes outreach activities to recruit eligible participants and establish partnerships in the community that support community awareness of services.
  • Ensures fidelity to National PAT model implementation standards. Participates in the process of continuous self-monitoring, reflection, and quality improvement to enhance program practices and maintain fidelity standards.
  • Participates in the process of ongoing professional development and meetings. Maintains comprehensive communication among all contacts within the agency, members, and external agencies. Models professional standards and ethics in accordance with EBF Code of Conduct, Strategic Plan, Mission, and Vision. 
  • Performs other duties as assigned.

Requirements

Minimum Requirements: High School diploma/GED and experience with working with families. Bachelor’s degree in Early Childhood Education, Early Childhood Special Education, Child & Family Studies, Social Work, Adult Education, or related field preferred. Bilingual (Spanish) required. 


Regulatory 

  • Must be at least 21 years of age. 
  • Current, unrestricted AZ driver’s license with no more than two (2) minor moving violations or one (1) accident within the past three (3 years). Three (3) years of driving experience required.
  • Must be able to pass a criminal background check.
  • Ability to obtain and maintain an AZ DPS Level 1 fingerprint clearance card (employer provides).
  • CPR and First Aid certification (Employer provides).
  • Successfully complete Parents as Teachers Foundational and Model Implementation training for certification for children ages 0-3 years and Foundational 2 certification for children ages 3-5 years within 1 year from start (employer provides).

Skills/Job Knowledge/Abilities: Able to establish and maintain a team atmosphere of communication and collaboration for all that reach out to the department and program. Must be self-directed and be sensitive to cultural and linguistic diversity. Excellent customer service in stressful situations. Maintains professional appearance, communications, and actions. Excellent interpersonal communication skills. Knowledge of infant/toddler typical and atypical development; family systems and group dynamics; principles of family-centered care; and Arizona’s Infant and Toddler Developmental Guidelines. Basic knowledge of First Things First and Parents as Teachers policies, practices, and procedures. Possess a high level of computer proficiency.


Working Conditions/Physical Requirements (with or without accommodation): Normal office environment with multiple interruptions in person and through computer and phone. Daily travel to family homes and community settings. Will work in outside and inside environments, as needed. Interacts with employees and members of the public on a daily basis. High activity environment with children and parent interactions. May lift normal office equipment and materials up to 25lbs. Visual acuity to read information from computer screens, forms, and to assess the wellbeing of the children. Able to speak clearly in conversations and general communications. Hearing ability for communication in persona, phone, and/or other electronic methods. Manual dexterity for typing and writing. Able to stoop, squat, reach, pull, push, stretch, ascend and descend stairs, stand and sit for long periods of time. May be required to work additional hours or days depending on circumstances.
