About the Role

Description

Position Overview:

This role assists individuals in navigating service systems, developing person-centered plans, and monitoring services to ensure health, safety, and meaningful outcomes for each person served as described in He-M 503.


Essential Responsibilities:

System Navigation:

• Identify the needs and support required for individuals by accessing appropriate assessment tools as needed.

• Provide information and assistance to individuals and their support teams on options to meet their needs.

• Work collaboratively with community service partners to facilitate connections on behalf of the individual

• Facilitate transition planning as required between waivers or services.

• Inform individuals of the appeals process


Person-Centered Planning:

• Identify individual needs/goals and preferences and champion those at all intersections of the process

• Coordinate the service planning process as outlined in He-M 503.

• Ensure all documentation is maintained, on time, and compliant with regulations outlined in He-M 503.

• Support the individual to facilitate the choice and selection of service providers to deliver services, conduct outreach on behalf of the individual

• Ensure adequate financial resources are secured for all requested services through budget development, accurate and timely authorization submission and room and board requests etc.

• In advance of the annual service planning meeting, consult with the individual and team members to understand the individual’s satisfaction with services, progress on their goals, and to identify any risk factors and plans to minimize them, as outlined in He-M 503, when applicable consult with the Human Right’s Committee through the formal process as outlined in He-M 504.


Monitoring Health, Safety and Outcomes:

• Conduct in-home visits and monthly contact at the frequency required regulation that:

• Monitor goal progression and address gaps between service delivered and expectations outlined in the service agreement.

• Assess the health and safety of the individual and address and report any concerns to appropriate parties and update HRST as needed

• Monitor the implementation of any risk management/behavioral support plans

• Submit documentation of these contacts and ensure timely and accurate billing is completed.

• Coordinate, complete, and document appropriate follow up related to incident reports and sentinel events, and participate in investigations as required

• Actively coordinate crisis situations including submitting crisis service funding requests to BDS.

• Ensure implementation and documentation of informed choice, freedom of choice, and protecting individuals rights.

Requirements

Qualifications:

Education and Experience:

• Associates Degree in Human Services or related field required; Bachelor’s preferred

• Minimum of 2 years' experience working with individuals with intellectual/developmental disabilities is required

• $100,000/300,000 Automobile Liability Insurance

• Valid driver's license and access to a safe operating vehicle during scheduled working hours
