Job Description

  • FTE: Part-time (0.7FTE) position available. Ongoing.
  • Location:  Based in and around the Toowoomba (homelands of the Jagera, Giabal, and Jarowair peoples) area with outreach required
  • Salary: Up to $51.74 per hour depending on experience and qualifications + 12% Super + Generous Salary Packaging benefits of up to $18,550

Join a passionate, multidisciplinary team delivering person-centred mental health care as part of the new Toowoomba Integrated Mental Health Hub, an innovative partnership between Each, Wellways, Lumsden Psychology, QPASTT and Goondir Health Services.

Your role, your impact
As a Clinical Care Coordinator, you’ll provide holistic, wrap-around support for people living with or at risk of mental illness. You’ll conduct bio-psychosocial assessments, develop individual care plans, and coordinate access to evidence-based interventions such as CBT, DBT, ACT and Motivational Interviewing.

This role is pivotal in ensuring seamless, integrated care that addresses both mental and physical health needs, empowering participants and their families with choice, control and connection to ongoing supports.

Key responsibilities:

  • Coordinate access to evidence-based psychological interventions and community supports
  • Deliver interventions including psychoeducation and evidence-based therapies (CBT, DBT, ACT, MI)
  • Identify and manage co-morbidities including physical health and alcohol or drug impacts
  • Support participants and families to actively engage in their recovery journey
  • Build and maintain partnerships with clinical and community stakeholders to ensure integrated service delivery
  • Promote a positive, collaborative team culture within the consortia and contribute to continuous quality improvement
  • Coordinate care across emergency departments, inpatient services, GPs and community mental health teams to ensure continuity and safety of care.

Who we’re looking for

  • Relevant experience in the mental health and welfare community sectors
  • Skilled in assessment, evidence-based interventions, and care coordination in complex mental health settings
  • Demonstrated experience supporting people with moderate to complex mental health needs
  • Knowledge of the social determinants of health and mental health recovery principles
  • Experience working with people from diverse backgrounds
  • Committed to trauma-informed, recovery-oriented, and culturally safe practice
  • Brings a proactive, compassionate, and non-judgmental approach to supporting participants and families

Qualifications

  • Qualified and experienced mental health professional (Social Worker or Occupational Therapist).
  • Eligible for registration with AHPRA or membership with a relevant professional body
  • Ability to travel across the Toowoomba region for outreach work (current driver’s licence required).
