We are a nonprofit organization provides supportive services focused on strengthening

families, children and individuals throughout the life cycle within the context of their unique needs. We offer a wide range of mental health and counseling services, social services, case management programs and safe and affordable housing to nearly 2,500 individuals annually. Jewish Family Services serves adults, children, seniors, individuals with disabilities, chronic mental health problems and substance use disorders and families with low household incomes.


The CCS Mental Health Professional is responsible for providing day to day oversight and consultation to Care Coordination staff, conducting intakes, completing assessments, participating in and/or developing recovery and discharge plans, and evaluating service plans according to the requirements of the Comprehensive Community Services program. Additionally, the CCS Mental Health Professional is responsible for leading the Care Coordination team in developing treatment plans that meet CCS consumers’ unique needs and recovery goals, enabling them to achieve their individual highest level of independent functioning.

Duties and Responsibilities

  • As a member of the recovery team, participate in the consumer assessment process, and recovery and discharge planning, and authorize consumer services.
  • Provide immediate clinical supervision of Care Coordinators.
  • Develop, monitor and ensure compliance with all administrative policies, procedures, state-specific regulatory requirements, contract requirements and service standards set forth by the Milwaukee County CCS program.
  • Oversee the performance of care coordination, substance abuse and clinical components of the CCS program, develop and implement program goals and objectives, assess program effectiveness, and gather program data for management, reporting and QA purposes.
  • Provide and facilitate services, as needed, including care coordination, intake and assessment activities.
  • Maintain full compliance with all documentation, reporting and service delivery requirements with related contracts, agreements, etc.
  • Intervene and support staff and/or consumers in crisis and develop plans to address crises as needed.
  • Supervise program personnel, provide orientation and training, plan work assignments, establish and maintain individual and program performance goals, maintain clinical integrity and complete and oversee staff performance evaluations; recruit, interview and hire job candidates.
  • Provide clinical supervision to include individual, group, team meeting, and side-by-side direct supervision as required.
  • Maintain and monitor credentialing status of CCS staff (including own status) to ensure continued compliance with underlying service contracts and regulations.
  • Provide oversight, management and reporting of consumer satisfaction with services provided within the program and identify and address any areas requiring improvement.
  • Maintain compliance with all levels and forms of consumer confidentiality.
  • Represent JFS and the CCS program to Milwaukee County, community groups, funding sources, contracting and regulatory agencies, referral sources and professional sources. Act as the liaison between the contracting agencies, other agencies and organizations, and JFS.
  • Maintain knowledge of and ability to work effectively with community based resources, referral networks, and public/private systems of care.
  • Provides operational support as needed.


Requirements

Possess a Master's Degree in Clinical Psychology, School or Educational Psychology, Counseling and Guidance, Counseling Psychology or Social Work preferred;

Have completed 3000 hours of supervised clinical experience where the majority of clients are children or adults with mental disorders or substance-use disorders;

Hold a valid Wisconsin license from one of the following Examining Boards: Social Work (preferred), Marriage and Family Therapy or Professional Counselors (LCSW, LMFT, LPC), and be in good standing with their Licensing Board;

Meet the requirements of the CCS Mental Health Professional as described in WI Chapter DHS 36.10 (b) (1) and (3).

Have at least 2 years of case management experience with adults and/or youth; CCS experience is preferred and at least 1 year of case management supervision experience with demonstrated increasing levels of successfully executed responsibility required.

Must drive to individual's homes at least 10% of the time, requiring valid driver’s license, access to reliable vehicle, and automobile insurance coverage throughout employment.

Knowledge, understanding, and/or willingness to learn Jewish culture and traditions and those of the Agency population as necessary.

Accept other duties and responsibilities as assigned.

Candidates must successfully pass a criminal background check with the Wisconsin Department of Justice and a caregiver background check with the Wisconsin Department of Health Services, the Department of Children and Families and the Department Safety and Professional Services.

Candidates must pass a Dru Sjodin National Sex Offender and Wisconsin Sex Offender Registry screening.

Documentation, or verified medical exemption, of immunizations and screening: Tuberculosis, Flu, Measles, Mumps, Rubella, and Varicella (Chicken Pox)
