Summary of Position

 

The Population Health and Wellness Nurse/Medical Assistant plays a critical role in the delivery of quality medical care within the center.  This position requires a person with strong clinical skills along with strong data collection/assessment and communication skills.  This person is responsible for delivering excellent, high-touch, primary care, and coordinating a patient centered care experience. The Population Health and Wellness Nurse/Medical Assistant provides care as described by the established policies and procedures of RCCHC.  This position will provide clinical care in collaboration with other partnerships to achieve positive patient outcomes.  The Population Health and Wellness Nurse/Medical Assistant will model behaviors consistent with the published values and the Code of Conduct of RCCHC.

Roanoke Chowan Community Health Center will provide open access, community-oriented primary care of the highest quality to all residents within our service area and surrounding counties, regardless of their ability to pay.

 

 

 

Minimum Qualifications

 

  • Must demonstrate competencies in clinical practice; 1-3 years in Clinical setting; preferably 1-3 years as Medical Assistant 
  • Works effectively with others and is able to organize workload, as well as assume responsibility 
  • Must be able to work independently, exercise appropriate decision making skills, and function effectively on a team
  • Must be able to speak and communicate clearly and effectively
  • Excellent organizational, leadership, verbal and written communication skills
  • Must be able to effectively handle multiple tasks and priorities in order to meet deadlines

 

Education: Graduation from Medical Assistant Certificate program.  

 

Experience: Requires two years clinical experience. Care management experience preferred. 

 

Certification(s)/Licensure: Requires a current Certified/Registered Medical Assistant. Health Care Provider CPR certification required. 

 

Knowledge:

  • Knowledge and thorough understanding of Federally Qualified Health Centers (FQHC’s) in regard to the Federal 330 Public Health Service Act
  • Knowledge of thorough understanding of the 340B program
  • Knowledge of computer systems and applications.

Essential Functions

 

  • Performs independent clinical functions using RCCHC established procedures, policies, and guidelines
  • Electronic health record proficiency
  • Strong communication skills and customer service orientation
  • Performs Annual Wellness Visit in accordance with payer and RCCHC guidelines
  • Gathers data/Assesses the healthcare, educational and psychosocial needs of the patient/family 
  • Investigates healthcare options and facilitates communication among the patient, primary care provider and other members of the healthcare team to eliminate barriers and identify interventions for treatment
  • Uses motivational interviewing and shared decision-making modalities as appropriate to enhance patient engagement
  • Educates/Reinforces education to patient, family and other health care members on the role and purpose of Care Coordination, its processes, disease/case management programs and outcomes and makes referral to appropriate Care Coordination services as needed.
  • Administers medications/immunizations as ordered by RCCHC providers, documenting accurately and observing patients for reactions
  • Answers and return patient phone calls and other inquiries to ensure accurate and timely communication is facilitated between all members of the care team
  • Completes paperwork for assigned providers as per RCCHC guidelines
  • Establishes  reason for patient’s visit, completing the rooming process per established guidelines and documenting accurate data in the EHR  
  • Assists patients in the preparation for the provider’s examination, including undressing, assisting to the table, and dressing
  • Accurately reviews and records medications and /or procedures as directed by the provider
  • Accurately executes technical procedures when ordered such as EKG’s, drawing blood, obtaining and preparing laboratory specimens
  • Maintains proper storage and handling of equipment, supplies, and medications
  • Maintains a clean and orderly clinical working atmosphere; adequately prepare exam rooms and properly equip laboratory
  • Works with patients and family in giving accurate, understandable instructions in diet, special care procedures, medication, patient education
  • Participates as an active member of the RCCHC team
  • Maintains compliance with workflows, policies and procedures as well as other activities to support PCMH model of care and accreditation
