Emergency Engagement Specialist, ESRD

Alliant Health Solutions, a "Best Place to Work" and "Healthiest Employer", seeks Emergency/Engagement Specialist (ESRD) to support educational programs, quality improvement activities, community outreach, and disaster preparedness and response activities that encourage patient and family-centered care and engagement and to improve care for ESRD patients across all Networks. Under the direction of the Executive Director and in collaboration with other departments, the Emergency/Engagement Specialist coordinates all Alliant ESRD Network outreach through the provision of technical assistance, education, and training to ESRD professionals, patients and their family members; leads in disaster preparedness activities within the renal community; and is the primary liaison to the Patient Advisory Committee(s) (PAC) and Facility Patient Representatives (FPR).
ESSENTIAL JOB FUNCTIONS:
Contract Management
- Develop and monitor programs with measurement and feedback components as required
- Submit contract requirements timely
- Lead in the coordination of Disaster Preparedness and Management
- Act as liaison with the ESRD patients, families and provider community of the Network and with the various ESRD organizations
- Provide technical assistance for patient engagement activities as specified in the Statement of Work
Program Management
- Assess needs, assets, and capacity for health education in the ESRD community
- Design data collection activities, surveys, and evaluate outcomes to improve program efficiency and effectiveness
- · Plan & implement health education & outreach for both patients, families and professionals Serve as the Disaster Preparedness liaison to:
- Emergency Coalitions, State Department of Health Services, Centers for Medicare and Medicaid Service (CMS) and the ESRD Kidney Health Analytics & Responsive Emergency Support (KHARES) contractor.
- Support drills and preparedness activities including disaster community and dialysis facilities.
- Manage EQRS reporting and compliance activities
Patient services
- Serve as primary liaison and staff to the Patient Advisory Committee, including member recruitment , Coordinate the activities of the facility patient representatives
- Assist with patient grievances and facility concerns as needed
- Other duties as assigned

KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge, skill and ability to work independently and collaborate with team and community
- Knowledge, skill and ability to communicate in verbal and written formats, including the ability to present information, facilitate meetings and other public speaking needs
- Proficiency in Microsoft Office including Word, Excel, PowerPoint, Outlook, Teams, and Internet
- Knowledge in the science of public health education, measurement and use of data
- Knowledge of electronic and print outreach tools and methodologies
- Ability to read, write, and speak English
- Ability to travel by driving or flying to Company locations, customer events or meetings or other locations as needed.
EDUCATION, EXPERIENCE, AND TRAINING:
Required:
- Bachelor’s degree required, health, marketing and/ or communications
- Healthcare experience
- Experience working with federal and state regulatory agencies, professional and community groups
Preferred:
- Knowledge of Medicare, dialysis/ESRD
- Previous experience in Disaster management
