About the Role
Position Summary:
The Staff Social Worker provides medically related social services to residents, ensuring that psychosocial needs are met and that residents attain or maintain their highest practicable well-being. This role advocates for residents' rights, supports discharge planning, and collaborates with interdisciplinary teams.
Key Responsibilities:
- Conduct psychosocial assessments and update care plans based on resident needs.
- Provide counseling and supportive services to residents and families.
- Assist with admission, discharge, and transfer processes, ensuring appropriate communication with physicians and families.
- Facilitate discharge planning, including coordination with community resources and services.
- Advocate for resident rights and provide education to staff on psychosocial issues.
- Document social services interventions and progress in resident records.
- Participate in interdisciplinary care conferences and survey preparation.
Skills & Competencies
- Knowledge of long-term care social work practices and regulations.
- Strong communication and counseling skills.
- Ability to work effectively with residents, families, and interdisciplinary staff.
- Organizational and problem-solving abilities.
Required:
- Bachelor's degree in Social Work or related field preferred.
Preferred:
- Experience in long-term care or healthcare social services.
Physical & Work Requirements
- Ability to move freely throughout the facility and interact with residents and families.
- Occasional lifting of up to 40 lbs and pushing wheelchairs as needed.
