Our goal was, and still is, to provide services regardless of the ability to pay, immigration status, or health insurance coverage. Our Impact: Today, BMS is dedicated to the cultural, economic, medical, and educational well-being of everyone in our community. We continue to advocate for increased healthcare access and inspire overall well-being throughout Brooklyn. Our Founder: Joseph Francois, former CEO of BCDC and founding Executive Director of BMS, was a passionate advocate for Brownsville, Ocean Hill, and East New York. About the Role

Description

The Physical Therapist (PT) provides evidence-based physical therapy services to patients across the lifespan, focusing on restoring functions, improving mobility, relieving pain, and preventing disability. The PT works with primary care providers, specialists, and interdisciplinary care teams to deliver coordinated, patient-centered care in an outpatient community health setting. The PT is compassionate, adaptable, and committed to serving diverse and medically underserved populations. 


This is an exempt, part-time opportunity (21 paid hours per week).


ESSENTIAL FUNCTIONS

Clinical Care 

  • Evaluate patients and develop individualized treatment plans based on clinical findings, functional goals, and patient needs 
  • Provide therapeutic interventions including therapeutic exercise, manual therapy, neuromuscular re-education, gait training, and patient education 
  • Monitor patient progress and modify treatment plans as clinically indicated 
  • Maintain accurate, timely, and compliant clinical documentation 

Care Coordination & Collaboration 

  • Collaborate with primary care providers, specialists, and care teams to ensure integrated and coordinated care 
  • Provide patient and family education on treatment plans, home exercise programs, injury prevention, and self-management strategies 
  • Participate in interdisciplinary case conferences and care planning as needed 

Quality, Compliance & Patient Safety 

  • Ensure compliance with federal, state, and organizational policies, including documentation and billing standards 
  • Participate in quality improvement initiatives, audits, and accreditation readiness activities 
  • Maintain licensure, certifications, and continuing education requirements 

Program Support & Professional Practice 

  • Contribute to the development and growth of rehabilitation services 
  • Support best practices in patient-centered, trauma-informed, and culturally responsive care 
  • Participate in staff meetings, trainings, and professional development activities 

REQUIRED QUALIFICATIONS AND COMPETENCIES 

  • Doctor of Physical Therapy (DPT) or master's degree in physical therapy from an accredited program 
  • Current, unrestricted New York State Physical Therapy license 
  • CPR/BLS certification 
  • Strong clinical, communication, and interpersonal skills 

PREFERRED QUALIFICATIONS AND COMPETENCIES 

  • Experience in an FQHC, community health center, outpatient clinic, or safety-net setting 
  • Experience working with diverse, underserved, or high-risk populations 
  • Bilingual (Spanish or other languages) preferred 
  • Experience with electronic health records (EHR) 

WORK ENVIRONMENT AND PHYSICAL DEMANDS 

  • Clinical setting with direct patient contact. 
  • Requires standing, walking, bending, and occasional lifting consistent with clinical practice. 
  • Ability to communicate, hear, see, and feel without limitations. 
  • May require extended periods of computer use for documentation.
