About the Role

Description

Are you a compassionate and clinically skilled Registered Nurse looking for an opportunity to make a meaningful impact in patients’ lives? Venture Forthe is seeking a Home Infusion Therapy Registered Nurse (RN) to provide high-quality infusion therapy services to patients in the comfort of their homes. This role combines hands-on clinical care, patient education, care coordination, and professional collaboration in a supportive and mission-driven environment.

What You'll Do: 

  • Provide specialized home infusion therapy services, including IV initiation, monitoring, dressing changes, blood draws, and pump programming.
  • Assess patients and develop individualized infusion plans of care.
  • Educate patients and caregivers on infusion therapy, medications, line care, and treatment expectations.
  • Collaborate with providers, pharmacies, and interdisciplinary care teams to ensure safe and effective care.
  • Monitor patients for infusion reactions, side effects, and clinical changes.
  • Maintain accurate and timely clinical documentation.
  • Support quality improvement initiatives and ongoing competency programs.
  • Assist with training and supervision of paraprofessional and nursing staff.


Work Environment: 

This position involves travel to patient homes and community settings throughout the service area. The role may require lifting and moving equipment and supplies in accordance with job requirements.  This position will also include some on call hours.

Requirements

What we are looking for: 

  • Current New York State Registered Nurse (RN) license  (required - candidates who do not have this experience will not be considered)
  • Graduate of an accredited nursing program  (required - candidates who do not have this experience will not be considered)
  • At least 1 year experience with infusion therapy which may include IV Therapy, PICC Lines, Central Line Care, Biologics, TPN, IV Antibiotics, and/or Hydration Therapy)  (required - candidates who do not have this experience will not be considered)
  • At least 1 year of Home Care experience (preferred but not required)
  • At least 1 year of hospital experience (preferred but not required)
  • Strong communication, organizational, and critical thinking skills
  • Ability to work independently in varied home settings
