About the Role

Case Manager – Community Treatment Order Program – East (Full Time Permanent)



 

The Role and Opportunity

The Canadian Mental Health Association (CMHA) – Toronto is looking for a Case Manager for the Community Treatment Order (CTO) Program, East Team. This intensive case management program supports individuals with severe mental illness who have been issued a CTO. Grounded in psychosocial rehabilitation principles, the program emphasizes recovery through consumer participation and collaboration. Services are provided to individuals aged sixteen and older who require support to remain in the community.


Salary: $54,943 – $60,203

Hours of Work: 35 per week

Location: 1200 Markham Rd., Suite 500, and in the community

Responsibilities

Reporting to the Program Manager, the Case Manager will:

  • Work with the client, CTO Coordinator, treatment team, Substitute Decision Maker (if applicable), and other stakeholders to develop and implement individualized service plans.
  • Support clients in meeting the obligations of their CTO and in accessingappropriate servicesand resources.
  • Conduct assessments, provide supportive counseling, skills teaching, and psychoeducation on illness, medications, and symptom management.
  • Build and maintain collaborative, respectful relationships with clients and their support systems.
  • Deliver direct service using a strength-based approach, monitor progress, and conduct follow-ups.
  • Advocate on behalf of clients and establish effective working relationships with community partners and service providers.

Experience & Education Required  

The ideal candidate will have:

  • An undergraduate degree or diploma in social services or a related field.
  • A minimum of three years’ experience in direct service delivery to adults with serious psychiatric disabilities (experience in community mental health is an asset).
  • Strong knowledge of crisis intervention/prevention, addictions, recovery-oriented practice, culturally competent and trauma-informed care, and severe/persistent mental illness.
  • A solution-focused approach, strong crisis management skills, and experience working with diverse communities from an anti-racism, anti-oppression, and non-homophobic lens.
  • The ability to work both independently and within a multidisciplinary team, with effective analytical, assessment, and decision-making skills.
  • A valid Ontario Driver’s License, access to a vehicle, andappropriate insurancecoverage.
