SOCIAL WORKER/DISCHARGE PLANNER, IDH

Description
Assists in providing services to patients, their families, or significant others who are in need of help with social, economic, or environmental problems resulting from, or related to their behavioral health diagnosis. Using creative communication strategies, the Social Worker / Discharge Planner will assist IDBH patients to anticipate, identify and overcome barriers within their journey through the healthcare system. The Social Worker / Discharge Planner works closely with the members of the multidisciplinary behavioral health team, including patients, families, physicians, nursing staff, and many other disciplines in the Iredell Health System involved in their care. Assists the patient in understanding their diagnosis, coordinating their treatment plans, seeking assistance from various appropriate social services and assisting the patient in communicating their treatment plan with their family and caregivers, documenting all interactions and outcomes. Partners with a network of community agencies to meet identified psychosocial needs of the patient. Has full access to patient health information.
Requirements
- Bachelor's Degree required. Bachelor's Degree in Social Work preferred. License or certification in Social Work preferred. MSW or Master in Counseling preferred. Education from an accredited institution required.
- At least one year of relatable behavioral health social work is required.
- At least one year experience in counseling is preferred.
- Experience in Patient Navigation and Discharge Planning is preferred.
- Understanding of principles and practice of social work. Must have working knowledge of community social and health agencies. Must have knowledge of human behavior and personality dynamics.
- BCLS certification.
