About the Role

Job Summary:
Certified Diabetes Educator or certification eligible RN, RD, Pharm D or Social Worker employed by Erlanger Medical Center is responsible for direct and/or indirect care of the patient under the supervision of a Physician or Nurse Practitioner/Physician Assistant.
This position is involved in a team-based approach to care. Team members are trained to meet the highest level of function for their role as per the State of Tennessee/Georgia guidelines. Is trained and assigned (as per their role and responsibilities) to coordinate care for patients and is trained and assigned to support patients/families/caregivers in self-management, self-efficacy and behavior change. Is trained and assigned (as per their role and responsibilities) to manage the practices� patient population. Participates in the practice's quality improvement process and performance evaluation. Teaches diabetes self-management in both individual and group settings.

Education:
Required:
Graduate from an approved program in dietetics/nutrition or a Registered Nurse or Certified Diabetes Educator. CDE eligible Pharm D or Social Worker.

Experience:
Required:
A minimum of three years of progressive experience in a hospital or clinic setting required. Experience in Insulin Pump Education

Preferred:
Experience in teaching diabetes education.

Current professional registration required. BLS prior to going to the unit/ department and maintained going forward

Department Position Summary:
Establishes and coordinates the diabetes self-management training for patients with diabetes. The majority of patients seen are adults. Participates in client centered case management and coordinates multidisciplinary plan of care.

The employee must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served in the clinic. The individual must possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs and to provide the care needed as described in the department's policies and procedures.

The employee must be able to demonstrate the knowledge and skills needed to provide education to adult learners.
Teaches diabetes self-management to individuals and classes. Helps manage outcomes. Assist with other activities that promote and encourage diabetes education as assigned.
