Low-barrier services are designed to reach disconnected lesbian, gay, bisexual, transgender, queer and questioning (LGBTQQ) youth ages 16-24. The program is rooted in community building, culturally responsive, trauma-informed, client-centered care, and harm reduction models. A high percentage of program participants have experienced physical and/or sexual abuse, homelessness, engagement with criminal justice and/or foster care systems, substance use/abuse, and/or mental health challenges.</span><span data-ccp-props="{}"> </span></p></div><div><p><span data-ccp-props="{}"> </span></p></div><div><p><strong><span lang="EN-US" xml:lang="EN-US" data-contrast="auto">Role Overview:</span></strong><span lang="EN-US" xml:lang="EN-US" data-contrast="auto"> Under the direction of the Director of Youth Services and the TAY Housing Manager, the TAY Service Navigator serves transitional age youth (TAY) in our youth drop in space who are experiencing homelessness with case management, resources, and referral services. This position also provides Youth Access Point services through partnerships with the San Francisco Department of Homelessness & Supportive Housing (HSH) and other stakeholders.</span><span data-ccp-props="{}"> </span></p></div></div><div><p><span lang="EN-US" xml:lang="EN-US" data-contrast="auto"> </span><span data-ccp-props="{"> </span></p></div><div><p><strong><span lang="EN-US" xml:lang="EN-US" data-contrast="auto">ESSENTIAL RESPONSIBILITIES</span> </strong></p></div></div><div><div><div><ul role="list"><li role="listitem" aria-setsize="-1" data-leveltext="" data-font="Symbol" data-listid="15" data-list-defn-props="{" data-aria-posinset="1" data-aria-level="1"><p><span lang="EN-US" xml:lang="EN-US" data-contrast="auto">Provide Youth Access Point services to transitional age youth experiencing homelessness by utilizing partnerships and community partners to screen and assess participants using standardized tools and protocols in the city’s Online Navigation and Entry (ONE) System to access housing programs within San Francisco including the Department of Homelessness and Supportive Housing.</span><span data-ccp-props="{}"> </span></p></li></ul></div><div><ul role="list"><li role="listitem" aria-setsize="-1" data-leveltext="" data-font="Symbol" data-listid="15" data-list-defn-props="{" data-aria-posinset="2" data-aria-level="1"><p><span lang="EN-US" xml:lang="EN-US" data-contrast="auto">Work as part of an interdisciplinary team to assess youth program participants' needs and connect youth with a wide range of necessary resources.
